精神疾病合并躯体疾病患者抗感染用药误区分析

摘　　要：目的:分析精神疾病合并躯体疾病患者抗感染用药中存在的问题和产生的原因,为提高此类患者合理应用抗生素提供参考。方法:利用医院药学管理系统,抽取2020年4月—2020年12月精神疾病合并躯体疾病出院患者1143例,评价抗感染治疗的合理性,分析不合理用药产生的原因及解决的办法。结果:1143例患者中,治疗性使用抗生素508例(44.44%),其中抗生素使用不合理116例(22.83%)。不合理用药类型涉及药物选择、联合用药、用法用量不适宜,用药依据不充分、疗程过长、改变治疗方案缺乏依据、抗感染治疗不足。结论:精神疾病并躯体疾病患者感染的诊断和治疗更复杂,抗感染治疗存在误区,需在正确的抗感染理念指导下,结合此类患者特殊病理生理状况,合理使用抗生素。Objective:To explore the misunderstanding of anti-infective medication in patients with mental disorders complicated with physical diseases to provide reference for improving the rational application of antibacterial drugs.Methods:A total of 1143 cases in patients with mental disorders complicated with physical diseases discharged from the psychiatric ward of a hospital from April to December 2020 were selected by the pharmacy management system of the hospital.The rationality of anti-infection treatment was analyzed and evaluated,and unreasonable factors were explored.Results:Among the 1143 patients investigated,508 cases were treated with antibacterial drugs,and the utilization rate of antibacterial drugs was 44.44%.Unreasonable use of antibiotics was 116 cases,accounting for 22.83%of the cases.Irrational drug use involves inadequate drug basis,inappropriate drug selection,drug combination,usage and dosage,too long course of treatment,lack of basis for changing treatment plan,and insufficient anti-infection treatment.Conclusion:The diagnosis and treatment of infection in patients with mental diseases combined with physical diseases investigated are more complicated,and there are misunderstandings in anti-infection treatment.Therefore,the rational use of antibacterial drugs should be guided by the correct anti-infection concept and combined with the special physiological conditions of these patients.
