背侧入路楔形植骨应用于背伸成角型桡骨远端骨折畸形愈合的效果观察

摘　　要：分析背伸成角型桡骨远端骨折畸形愈合患者采用背侧入路楔形植骨的效果。方法:选取我院2017年3月-2019年3月就诊的132例背伸成角型桡骨远端骨折畸形愈合患者为研究对象,按照治疗方式不同分为2组展开回顾性分析,将采用掌侧入路截骨矫形术治疗的66例患者纳入对照组,将采用背侧入路楔形植骨治疗的66例患者纳入观察组,对比2组手术情况、术后影像学检查结果、腕关节功能、腕关节活动度及并发症发生情况。结果:2组患者的手术时间、术后截骨愈合时间以及治疗后腕关节功能、活动幅度、疼痛评分、屈伸活动度、旋转活动度对比无显著差异(P>0.05),对照组握力强度与腕关节改良Mayo总分显著高于观察组(P<0.05);治疗后2组患者的掌倾角降低,尺偏角增加,桡腕关节面移位缩短,组内对比均有显著差异(P<0.05),但组间对比无显著差异(P>0.05);对照组术后肢体麻木发生率显著高于观察组(P<0.05)。结论:背侧入路楔形植骨治疗背伸成角型桡骨远端骨折畸形愈合效果确切,与掌侧入路疗效相当,但安全性更佳,仅在术后腕关节握力强度上有些微差异,适用于对握力要求不高、年龄较大的患者,临床中可根据患者实际情况科学选择适合的治疗方式。To analyze the effect of wedge bone grafting of dorsal approach in patients with posterior distal radial bone fracture malunion.Methods:Select 132 patients with distal radial fracture from March 2017 to March 2017-2019,divided into two groups,66 patients with palar approach osteotomy in the control group,66 pa-tients in the observation group,comparing the surgical conditions,postoperative imaging results,wrist function,wrist mobility and complications.Results:The operative time,postoperative healing time,wrist function,activity range,pain scores,flexion,and rotational activity were not significantly different(P>0.05).significantly higher in the control group(P<0.05)(P<0.05),but (P>0.05),and significantly higher than in the control group(P<0.05).Conclusion:Dorsal approach wedge bone graft treatment back extension Angle distal radial fracture deformi-ty healing effect is exact,and the volar approach efficacy but better safety,only in the postoperative wrist grip strength some slight differences,suitable for grip strength requirement is not high,older patients,clinical patients ac-cording to the actual situation of scientific choice for treatment.
