柳宝诒辨治伏气温病经验浅析

摘　　要：晚清名医柳宝诒著有《温热逢源》《惜余医案》《柳选四家医案》等书,并提倡用养阴托邪一法治疗伏气温病,在温病学的发展过程中起到了重要作用。柳宝诒强调伏邪为病,认为“冬伤于寒”“冬不藏精”是导致伏气化温的原因。他驳斥其他医家“寒毒藏于肌肤”“鼎立三纲”等观点,引《黄帝内经》说明伏邪不伏别经而必伏少阴的合理性。辨证上柳宝诒以张仲景六经辨证为纲,兼参卫气营血,募原三焦等辨证方式。治疗上柳宝诒总以祛邪养阴为要,并根据肾气之亏虚分为三类:肾气始虚,泄热透邪;肾气虚甚,养阴托邪;肾阳虚馁,温托寒邪。LIU Baoyi,a famous doctor in the late Qing Dynasty,has written books such as Wen Re Feng Yuan,Xi Yu Medical Records,Four Medical Records Chosen by LIU Bao-yi,and has pioneered the method of nourishing Yin so as to dispelling pathogenic factors in the treatment of latent Qi febrile diseases,which has played an important role in the development of febrile diseases.LIU emphasizes that the febrile disease is caused by insidious pathogen,and the cold evil in winter and deficiency of kidney Qi are the two main factors.LIU cites Inner Canon of Huangdi to support his point,that insidious pathogen is hidden in Shaoyin meridian,instead of the other meridians.Meanwhile,he seriously refutes other doctors views,such as insidious pathogen hidden in skin,bone marrow,or other places.For syndrome differentiation,LIU combined six-channel pattern identification,with defense-Qi-construction and blood pattern identification,and syndrome differentiation of triple energizer.In the treatment of latent Qi febrile diseases,he focuses on dispelling pathogenic factors and nourishing Yin.According to the deficiency of kidney Qi,three methods of treatment are divided.With enough kidney Qi,he paid attention to dispelling pathogenic factors;instead,with insufficient kidney Qi,nourishing Yin to supplement the deficiency in order to expel the evil.What s more,with insufficient kidney Yang,warming Yang to strengthen the healthy Qi.
