摘  要:目的了解中国18~95岁成年人超重肥胖流行现状,探讨研究中国成年居民午睡习惯、夜间睡眠时长与超重肥胖的关联,为采取相应的干预措施提供科学依据。方法数据来源中国家庭追踪调查(china family panel studies,CFPS)数据,于2020-06/08采用内隐分层、多阶段整群随机抽样的方法以中国25个省/市/自治区21168名成人居民为研究对象,通过问卷调查收集人口学、成人午睡习惯和夜间睡眠时长、行为生活方式等信息,体格测量获得成人的身高和体重。超重肥胖的影响因素进行描述性分析和单因素分析,并采用多因素Logistic回归分析睡眠与成人超重肥胖的关联。结果纳入有效样本共21168例,超重率为29.97%,肥胖率为8.90%。研究对象的性别、年龄、城乡、婚姻、受教育水平、自评健康、有无慢性病、每周锻炼频率、蔬果摄入、吸烟、饮酒、午睡习惯、夜间睡眠时长情况对超重、肥胖的发生有显著影响(P<0.05)。多因素Logistic回归分析结果显示,校正混杂因素后,与无午睡习惯组相比,有午睡习惯组增加14.1%超重风险(OR=1.141,95%CI:1.071~1.216)和16.2%肥胖风险(OR=1.162,95%CI:1.049~1.287);与夜间睡眠时长<7 h组相比,夜间睡眠时长7~8 h组降低10.9%超重风险(OR=0.891,95%CI:0.832~0.953)、降低21.9%肥胖风险(OR=0.781,95%CI:0.716~0.851),夜间睡眠时长>8 h组降低10.9%超重风险(OR=0.781,95%CI:0.832~0.953)、降低31.3%肥胖风险(OR=0.687,95%CI:0.597~0.790)。结论成年居民无午睡习惯、夜间睡眠时长≥7 h可能是超重、肥胖的保护因素。午睡习惯与超重肥胖之间的关系及其机制未来仍需进一步的研究。Objective To understand the status of overweight and obesity epidemic among Chinese adults aged 18-95 and to explore the associations between nap habits and nocturnal sleep duration with overweight or obesity among Chinese adult residents,so as to provide the scientific reference for corresponding appropriate intervention measures.Methods The data were from China Family Panel Studies(CFPS).From June to August 2020,a questionnaire of 21168 adult residents was completed in 25 provinces/cities/autonomous regions of China by implicit stratified and multi-stage cluster random sampling.The information of demography,siesta habits,night sleep duration,behavior and lifestyle,weight and height were collected via questionnaire and anthropometric measurement.Descriptive analysis and univariate analysis were used to analyze the influencing factors of overweight and obesity,and multivariate Logistic regression was used to analyze the association of sleep with overweight and obesity.Results Among the 21168 participants,the prevalence rates of overweight and obesity were 29.97%and 8.90%,respectively.There were significant differences in gender,age,urban/rural area,marriage,education,self-rated health,chronic diseases,exercise frequency weekly,intake of vegetables and fruits,smoking,drinking,midday napping duration and nocturnal sleep duration among the normal,overweight and obese groups(all P<0.05).Multivariate logistic regression analysis showed that after adjusting for confounding factors,compared with the non nap habit group,the nap habit group increased the risk of overweight by 14.1%(OR=1.141,95%CI:1.071~1.216)and obesity by 16.2%(OR=1.162,95%CI:1.049~1.287);Compared with the group with night sleep duration<7 h,the group with night sleep duration 7~8 h reduced the risk of:overweight by 10.9%(OR=0.891,95%CI:0.832~0.953),obesity by 21.9%(OR=0.781,95%CI:0.716~0.851),the group of night sleep duration>8 h reduced the risk of overweight by 10.9%(OR=0.781,95%CI:0.832~0.953),obesity by 31.3%(OR=0.687,95%CI:0.597~0.790).Conclusions
