摘　　要：视网膜血管疾病(RVD)是最常见的眼底血管性疾病,主要包括糖尿病视网膜病变(DR)、视网膜血管阻塞和血管炎等疾病。RVD病理机制复杂,为临床治疗带来一定的挑战。自古以来,中医药对RVD具有独特的认识和治疗方法,主要包括五轮辨证和眼底血证分阶段论治等,而相应治疗方法也多种多样。笔者根据RVD的临床特点,认为其病因病机早期主要为热郁毒聚,中期主要为脉滞络痹,并通过广泛临床实践,获得了较好的疗效。本文就RVD的中医认识、不同病理阶段的中医辨证、中药治疗方法和现代药理学研究进行探讨,并通过临床案例介绍进一步阐明观点,以期建立系统的RVD辨证治疗方法。Retinal vascular diseases(RVD)are the most common fundus vascular diseases,mainly including diabetic retinopathy(DR),retinal vascular occlusion and vasculitis.The pathological mechanism of RVD is complex,which brings certain challenges to clinical treatment.Since ancient times,Traditional Chinese Medicine(TCM)has unique understanding and treatment methods for RVD,mainly including five rounds of syndrome differentiation and treatment of fundus blood syndrome by stages,and the corresponding treatment methods are also diverse.According to the clinical characteristics of RVD,the author believes that the etiology and pathogenesis of RVD is mainly heat stagnation and toxin accumulation in the early stage and pulse stagnation and collateral obstruction in the middle stage.Moreover,it has achieved good results through extensive clinical practice.This paper discusses the understanding of TCM on RVD,syndrome differentiation at different pathological stages,treatment methods of TCM and modern pharmacological research,and further clarifies the point of view through the introduction of clinical cases in order to establish a systematic syndrome differentiation and treatment method for RVD.
