黄芪桂枝五物汤加味泡洗包在乳腺癌患者紫杉类药物相关周围神经毒性中的临床观察

摘　　要：目的分析黄芪桂枝五物汤加味泡洗包在乳腺癌患者紫杉类药物相关周围神经毒性中的临床疗效。方法采用便利抽样法,选取2021年1月至2021年12月中国医学科学院北京协和医学院肿瘤医院内科病房收治的60例紫杉醇化疗后发生周围神经毒性的患者为研究对象,依据便利抽样法分为试验组与对照组,每组各30例。对照组患者采取常规护理措施,试验组患者在对照组基础上联合使用黄芪桂枝五物汤加味泡洗包治疗。比较两组患者的临床疗效以及干预前后中医证候积分量化表、化疗药物周围神经毒性评价量表和心理状态评分量表评分。结果试验组患者治疗总有效率显著高于对照组(P<0.05)。试验组干预后患者下肢麻木、四肢疼痛、倦怠乏力的中医证候评分,四肢麻木、四肢刺痛、四肢不适、双脚受限的周围神经毒性评分,焦虑、抑郁评分均显著低于本组干预前和对照组同期评分(均P<0.05)。结论黄芪桂枝五物汤加味泡洗包能改善乳腺癌患者紫杉类药物相关周围神经毒性的中医临床症状。Objective To evaluate the clinical efficacy of Huangqi Guizhi Wuwu Decoction plus flavor bubble washing bag in the treatment of paclitaxel related peripheral neurotoxicity in patients with breast cancer.Method A total of 60 patients with peripheral neurotoxicity after paclitaxel chemotherapy who were admitted to the internal medicine ward in the Cancer Hospital,Chinese Academy of Medical Sciences from January 2021 to December 2021 were selected and divided into experimental group and control group with 30 cases in each group according to the simple random number table.Control group patients were treated with routine nursing measures,and experimental group patients were treated with Huangqi Guizhi Wuwu Decoction plus flavor bubble washing bag on the basis of control group.The scores of the TCM syndrome integral quantification table,the treatment of cancer quality of life questionnaire-chemotherapy-induced peripheral neuropathy twenty-item scale(QLQ-CIPN20)and mental state rating scale were compared between the two groups before and after treatment.Result The total effective rate of patients in experimental group was significantly higher than that in control group(P<0.05).After intervention,the syndrome scores of lower limb numbness,limb pain,burnout and fatigue,the peripheral neurotoxicity scores of limb numbness,limb tingling,limb discomfort and feet limitation,and the anxiety and depression scores of patients in experimental group were all significantly lower than those before treatment in the same group(all P<0.05),and all were significantly lower than the scores of control group in the same period(all P<0.05).Conclusion The Huangqi Guizhi Wuwu Decoction plus flavor bubble washing bag can improve the clinical symptoms of peripheral neurotoxicity associated with paclitaxel in breast cancer patients.
