育龄期癌症女性病人生育忧虑发生现状的系统评价

摘　　要：目的:了解育龄期癌症女性病人生育忧虑发生现状,为相关干预提供依据。方法:检索Web of Science、PubMed、Embase、Cochrane Library、EBSCO、中国知网、维普、万方数据库、中国生物医学文献数据库自建库至2022年4月发表的育龄期癌症女性病人生育忧虑相关文献,采用Stata 16.0软件进行Meta分析。结果:共纳入27篇文献,6315例育龄期癌症女性病人。Meta分析结果显示,育龄期癌症女性病人生育忧虑评分为57.92[95%CI(56.41,59.43)]分,得分较高的两个维度为子女健康10.84[95%CI(10.57,11.11)]分和自身健康10.62[95%CI(10.25,10.98)]分。亚组分析显示,无子女、有生育意愿、文化程度高、月收入低、未婚、仅育女孩、联合采用放化疗、癌症分期Ⅰ期、甲状腺癌、中国病人生育忧虑更重。结论:育龄期癌症女性病人生育忧虑水平偏高,主要忧虑子女与自身健康。婚育、经济文化、疾病治疗、国家等特征不同的病人,生育忧虑水平存在差异。Objective:To understand the status of reproductive concerns of female cancer patients in childbearing age,and to provide the basis for relevant intervention.Methods:Literature on reproductive concerns of female cancer patients in childbearing age published from establishment of the database to April 2022 was retrieved from Web of Science,PubMed,Embase,Cochrane Library,EBSCO,CNKI,VIP,Wanfang Database and China BioMedical Literature Database.Stata16.0 software was used for Meta-analysis.Results:A total of 27 studies,involving 6315 female cancer patients in childbearing age.The results of Meta-analysis showed that the score of reproductive concerns of female cancer patients in childbearing age was 57.92[95%CI(56.41,59.43)],and the two dimensions with high scores were children’s health 10.84[95%CI(10.57,11.11)]and their own health 10.62[95%CI(10.25,10.98)].Subgroup analysis showed that childless,having fertility intention,high education level,low monthly income,unmarried,having only girls,combined radiotherapy and chemotherapy,cancer stageⅠ,thyroid cancer and Chinese patients had higher concerns.Conclusion:Female cancer patients in childbearing age have high reproductive concerns,mainly concerned about their children and their own health.Patients with different characteristics,such as marriage and childbearing,economy and culture,disease treatment,and country,have different levels of reproductive concerns.
