摘 要:目的:探讨基于时机理论的赋能教育在肝硬化合并上消化道出血病人中的应用效果。方法:选取2020年1月—2021年12月医院消化内科收治的肝硬化合并上消化道出血病人94例,应用随机数字表法将病人分为观察组47例和对照组47例。对照组行常规健康指导,观察组实施基于时机理论的赋能教育,比较两组病人干预前后焦虑与抑郁情绪、自我效能、遵医行为及预后情况。结果:干预后观察组病人焦虑自评量表(SAS)、抑郁自评量表评分(SDS)明显低于对照组(P<0.001),自我效能、遵医行为评分明显高于对照组(P<0.001)。干预后观察组病人再出血发生率、再入院率明显低于对照组(P<0.05),而观察组病人护理满意率明显高于对照组(P<0.05)。结论:基于时机理论的赋能教育可减轻肝硬化合并上消化道出血病人的不良情绪,提高病人遵医行为,有效降低再出血发生率及再入院率,提高病人满意度。Objective:To explore the application effect of empowerment education based on timing theory in patients with liver cirrhosis complicated with upper gastrointestinal bleeding.Methods: A total of 94 patients with liver cirrhosis complicated with upper gastrointestinal bleeding admitted to the gastroenterology department of the hospital from January 2020 to December 2021 were selected and divided into observation group(47 cases) and control group(47 cases) by random number table method.The control group received routine health guidance, and the observation group received empowerment education based on timing theory.Anxiety and depression, self-efficacy, compliance behavior and prognosis were compared between the two groups before and after intervention.Results:After intervention, the scores of Self-rating Anxiety Scale(SAS) and Self-rating Depression Scale(SDS) in the observation group were significantly lower than those in the control group(P<0.001),while the scores of self-efficacy and compliance behavior were significantly higher than those in the control group(P<0.001).After intervention, the rebleeding rate and readmission rate of observation group were significantly lower than those of control group(P<0.05),and the nursing satisfaction rate of observation group was significantly higher than that of control group(P<0.05).Conclusion:The empowerment education based on timing theory can alleviate the negative emotions of patients with liver cirrhosis complicated with upper gastrointestinal bleeding, improve the patients′ compliance behavior, effectively reduce the incidence of rebleeding and readmission, improve patients′ satisfaction.
