痛痒消洗剂对肛周脓肿术后患者的临床疗效

摘　　要：目的 考察痛痒消洗剂对肛周脓肿术后患者的临床疗效。方法 110例患者随机分为对照组和观察组,每组55例,对照组采用常规治疗,观察组在对照组基础上加用痛痒消洗剂。检测临床疗效、IL-10、HMGB1、IL-5、sICAM-1、VEGF、ColⅢ、bFGF、水肿评分、肉芽生长评分、VAS评分、腐肉完全脱落时间、创面愈合时间、肛管最大收缩压、直肠肛管抑制反射阈、肛管静息压、SF-36评分变化。结果 观察组总有效率高于对照组(P<0.05)。术后,2组IL-5、HMGB1、sVCAM-1、水肿评分、VAS评分降低(P<0.05),IL-10、VEGF、ColⅢ、bFGF、肉芽生长评分、肛管最大收缩压、直肠肛管抑制反射阈、肛管静息压、SF-36评分升高(P<0.05),以观察组更明显(P<0.05),观察组腐肉完全脱落时间、创面愈合时间短于对照组(P<0.05)。结论 痛痒消洗剂可调节肛周脓肿术后患者Th1、Th2细胞免疫,抑制机体炎症,提高VEGF、ColⅢ、bFGF水平,缓解水肿和疼痛,促进肉芽生长和创面愈合,改善肛门功能和生活质量。AIM To investigate the clinical effects of Tongyangxiao Lotion on postoperative patients with perianal abscess.METHODS One hundred and ten patients were randomly assigned into control group(55 cases) for intervention of conventional treatment, and observation group(55 cases) for intervention of both Tongyangxiao Lotion and conventional treatment. The changes in clinical efficacy, IL-10, HMGB1, IL-5, sICAM-1, VEGF, Col Ⅲ, bFGF, edema score, granulation growth score, VAS score, carrion complete sloughing time, wound healing time, maximal anal squeezing pressure, rectal anal canal inhibitory reflex threshold, anal canal resting pressure and SF-36 score were detected.RESULTS The observation group demonstrated higher total effective rate than the control group(P<0.05). After the operation, the two groups displayed decreased IL-5, HMGB1, sVCAM-1, edema score and VAS score(P<0.05), and increased IL-10, VEGF, Col Ⅲ, bFGF, granulation growth score, maximal anal squeezing pressure, rectal anal canal inhibitory reflex threshold, anal canal resting pressure and SF-36 score(P<0.05), especially for the observation group(P<0.05), and the observation group exhibited shorter carrion complete sloughing time and wound healing time than the control group(P<0.05).CONCLUSION For the patients with perianal abscess, Tongyangxiao Lotion can regulate Th1 and Th2 cell immunities, inhibit inflammation, enhance VEGF, Col Ⅲ and bFGF levels, relieve edema and pain, promote granulation growth and wound healing, and improve anal function and life quality.
