摘　　要：目的探讨基于温柔分娩理念的助产士陪伴结合产时自由体位干预对初产妇产痛程度、分娩自控感及分娩信心的影响。方法选取2020年6月至2021年6月于我院分娩的400例初产妇作为研究对象,以随机数字表法将其分为对照组(常规助产护理干预)和观察组(在对照组基础上应用基于温柔分娩理念的助产士陪伴结合产时自由体位干预),各200例。比较两组的干预效果。结果观察组的现实疼痛强度量表(PPI)、言语反应量表(VRS)及视觉模拟评分法(VAS)评分低于对照组(P<0.05)。观察组的分娩控制量表(LAS)评分高于对照组(P<0.05);干预后,观察组简化中文版分娩自我效能量表(CBSEI-C32)的期望效能、结果效能评分高于对照组(P<0.05)。观察组的剖宫产、会阴侧切、会阴裂伤、产后出血、胎儿窘迫及新生儿窒息发生率均低于对照组(P<0.05)。结论基于温柔分娩理念的助产士陪伴结合产时自由体位干预应用于初产妇中,有助于减轻产痛,提高分娩自控感和分娩信心。Objective To explore the effects of midwife companion based on the concept of gentle childbirth combined with free postural intervention during intrapartum on the degree of labor pain,self-control of childbirth and confidence of childbirth in primipara.Methods A total of 400 primiparas who gave birth in our hospital from June 2020 to June 2021were selected as the research objects and divided into control group(routine midwifery nursing intervention)and observation group(midwife companion based on the concept of gentle childbirth combined with free postural intervention during intrapartum based on the control group)by random number table method,with 200 cases in each group.The intervention effects of the two groups were compared.Results The Present Pain Intensity(PPI),Verbal Rating Scale(VRS)and Visual Analogue Scale(VAS)scores in the observation group were lower than those in the control group(P<0.05).The score of Labour Agentry Scale(LAS)in the observation group was higher than that in the control group(P<0.05);after intervention,the expected efficacy and outcome efficacy scores of Simplified Version of Childbirth Self-Efficacy Inventory(CBSEI-C32)in the observation group were higher than those in the control group(P<0.05).The incidences of cesarean section,lateral perineal incision,perineal laceration,postpartum hemorrhage,fetal distress and neonatal asphyxia in the observation group were lower than those in the control group(P<0.05).Conclusion Midwife companion based on the concept of gentle childbirth combined with free postural intervention during intrapartum applied in primipara can help reduce labor pain,and improve the self-control of childbirth and confidence of childbirth.
