温针灸联合双氯芬酸钠及秋水仙碱治疗痛风性膝关节炎临床观察

摘　　要：目的探讨温针灸联合双氯芬酸钠及秋水仙碱治疗痛风性膝关节炎(GKA)的临床疗效。方法选取医院2021年11月至2022年10月收治的GKA患者102例,按随机数字表法分为观察组和对照组,各51例。两组患者均口服双氯芬酸钠缓释片及秋水仙碱片,观察组患者加用温针灸治疗,两组均连续治疗2周。结果观察组总有效率为96.08%,显著高于对照组的78.43%(P<0.05);与对照组比较,观察组患者治疗后的血浆黏度及全血高、中、低切黏度均显著降低(P<0.05),红细胞沉降率和血清肿瘤坏死因子-α、C反应蛋白、尿酸水平及视觉模拟量表评分均显著降低,Lysholm膝关节评分量表评分显著升高(P<0.05);观察组与对照组不良反应发生率相当(15.69%比13.73%,χ^(2)=0.078,P>0.05)。结论温针灸联合双氯芬酸钠及秋水仙碱治疗痛风性膝关节炎,能有效改善患者血液流变学指标和膝关节功能,下调血清炎性因子水平。Objective To investigate the clinical efficacy of warming acupuncture-moxibustion combined with diclofenac sodium and colchicine in the treatment of gout knee arthritis(GKA).Methods A total of 102 patients with GKA admitted to the hospital from November 2021 to October 2022 were selected and randomly divided into the observation group and the control group,with51 cases in each group.The patients in the two groups were given Diclofenac Sodium Sustained-Release Tablets and Colchicine Tablets orally,on this basis,the patients in the observation group were treated with warming acupuncture-moxibustion.Both groups were treated for two weeks.Results The total effective rate in the observation group was 96.08%,which was significantly higher than 78.43%in the control group(P<0.05).Compared with those in the control group,the plasma viscosity and whole blood high-,medium-,and low-shear viscosity in the observation group were significantly lower after treatment(P<0.05),the levels of erythrocyte sedimentation rate,tumor necrosis factor-α(TNF-α),C-reactive protein(CRP),uric acid(UA)and the Visual Analogue Scale(VAS)score in the observation group were significantly lower after treatment(P<0.05),while the Lyshom Knee Rsting System(LKRS)score in the observation group was significantly higher after treatment(P<0.05).The incidence of adverse drug reactions in the observation group was similar to that in the control group(15.69%vs.13.73%,χ^(2)=0.078,P>0.05).Conclusion Warming acupuncture-moxibustion combined with diclofenac sodium and colchicine in the treatment of GKA can effectively improve the hemorheology indexes and knee joint function of patients,and reduce the level of serum inflammatory factors.
