基于加权TOPSIS法的围术期注射用帕瑞昔布钠合理用药评价

摘　　要：目的为围术期合理使用注射用帕瑞昔布钠提供参考。方法以专家共识、临床指南、注射用帕瑞昔布钠药品说明书及相关文献为依据,制订该药围术期合理用药评价细则,并组织处方点评小组成员与临床药师、部分临床科室临床医学专家共同讨论修订。利用医院临床药学管理系统提取医院2020年1月至2021年6月围术期使用该药的病历126份,采用前述评价细则评价,并以评价结果构建数据库,使用属性层次模型赋权法确定各指标权重;基于加权优劣解距离(TOPSIS)法计算相对接近度(C_(i)),分析该药的用药合理性。结果9个纳入指标中,相对权重最高的为用法用量(0.1402),最低的为溶剂/疗程(0.0841)。126例病例的Ci为52.80%~100%,其中用药不合理(Ci<60%)率为7.94%,基本合理(60%≤C_(i)≤80%)率为45.24%,合理(80%≤C_(i)≤100%)率为46.82%。结论所建立的评价细则操作简便,结果直观可靠、清晰扼要,可用于评价注射用帕瑞昔布钠的用药合理性。该院围术期注射用帕瑞昔布钠的使用基本合理。Objective To provide a reference for the rational use of Parecoxib Sodium for Injection during the perioperative period.Methods Based on the consensus of experts,clinical guidelines,package insert of Paroxycoxib Sodium for Injection and relevant literature,the detailed rules for the evaluation of the rational use of the drug during the perioperative period were formulated,and the members of the prescription review group were organized to discuss and revise the rules with clinical pharmacists and clinical medical experts in some clinical departments.A total of 126 medical records of Paroxycoxib Sodium for Injection during the perioperative period were extracted from the Prescription Automatic Screening System(PASS)of the hospital from January 2020 to June 2021,and the established evaluation rules were used for evaluation to establish the database,and the weights of each index were determined by the attribute hierarchical model weighting method.The relative closeness(C_(i))was calculated based on the Technique for Order Preference by Similarity to an Ideal Solution(TOPSIS)method to analyze the rational use of the drug.Results Among the nine included indicators,the highest relative weight was the usage and dosage(0.1402),and the lowest was the solvent/course of treatment(0.0841).The 126 patients′Ciranged from 52.80% to 100%,of which the irrational rate(Ci<60%)was 7.94%,the basically rational rate(60%≤C_(i)<80%)was 45.24%,and the rational rate(C_(i)≥80%)was 46.82%.Conclusion The established evaluation rules are easy to operate,and the results are intuitive,reliable,clear and concise,which can be used to evaluate the rationality of drug use of Paroxycoxib Sodium for Injection.The use of Paroxycoxib Sodium for Injection during the perioperative period is basically reasonable.
