摘  要:为了解宫颈癌组织叉头翼状螺旋转录因子(FOXP3)、程序性死亡受体-l(PD-1)、程序性死亡受体配体-1(PD-L1)蛋白表达与宫颈癌发生发展的关系,选取宫颈癌患者70例、宫颈上皮内瘤变(CIN)患者50例的临床资料,将其分别纳入宫颈癌组和CIN组.比较宫颈癌组与CIN组、HPV感染组(26例)与无HPV感染组(44例)宫颈癌组织FOXP3、PD-1、PD-L1蛋白的表达情况及其与病理特征的关系,采用多因素Logistic回归分析法分析宫颈癌组织中3种蛋白阳性表达的相关危险因素.结果发现,宫颈癌组PD-1蛋白的阳性表达率高于CIN组的数值(P<0.05);HPV感染组宫颈癌组织FOXP3、PD-1、PD-L1蛋白的免疫组化评分均高于无HPV感染组(P<0.05)的评分.PD-1蛋白表达与PD-L1蛋白表达存在明显的相关性(P<0.05).FIGO分期的Ⅲ期或Ⅳ期、组织学分级中低分化、淋巴结转移中有淋巴结转移者FOXP3、PD-1、PD-L1蛋白的阳性表达率较高(P<0.05).多因素Logistic回归分析结果显示,FIGO分期为Ⅲ期或Ⅳ期、组织学分级为低分化、有淋巴结转移为宫颈癌组织3种蛋白阳性表达的独立危险因素(P<0.05).总之,宫颈病变与HPV感染均可导致FOXP3、PD-1、PD-L1蛋白呈高表达状态,3种蛋白与宫颈癌的发生发展密切相关,在反映患者病情、预后方面具有一定的价值.In order to explore the relationship of expression of fork wing spiral transcription factor(FOXP3),programmed death receptor-l(PD-1)and programmed death receptor ligand-1(PD-L1)protein with cervical cancer,the clinical data of 70patients with cervical cancer and 50 patients with cervical intraepithelial neoplasia(CIN)were retrospectively selected,and they were included in the cervical cancer group and the CIN group respectively.The expression of FOXP3,PD-1 and PD-L1protein in cervical cancer tissues were compared among the cervical cancer group,the CIN group,the HPV infection group(26cases)and the non-HPV infection group(44 cases),and their relationship with pathological features were analyzed.The risk factors of FOXP3,PD-1 and PD-L1 protein positive expression in cervical cancer tissues were analyzed by multivariate Logistic regression analysis.The results show that the positive expression rate of PD-1 protein in the cervical cancer group was higher than that in the CIN group(P<0.05).Immunohistochemical scores of FOXP3,PD-1 and PD-L1 protein in cervical cancer tissues of the HPV infection group were higher than those of the non-HPV infection group(P<0.05).There was a significant correlation between the expression of PD-1 protein and PD-L1 protein(P<0.05).The positive expression rate of FOXP3,PD-1 and PD-L1 protein was higher in the patients with stageⅢorⅣof FIGO stage,low differentiation in histological grade and lymph node metastasis(P<0.05).Multivariate Logistic regression analysis showed that FIGO stage was stageⅢorⅣ,histological grade was poorly differentiated,and lymph node metastasis were independent risk factors for the positive expression of the three proteins in cervical cancer(P<0.05).Overall,cervical lesions and HPV infection can lead to high expression of FOXP3,PD-1 and PD-L1 protein.The three kinds of protein were closely related to the occurrence and development of cervical cancer,and they had certain value in reflecting the patient′s condition and prognosis.
