摘  要:目的探讨内源性硫化氢(H2S)、尿肝素结合蛋白(HBP)、髓样细胞触发受体1(sTREM-1)在尿源性脓毒血症中的表达及相关性研究.方法选取2019年5月至2021年5月在本院诊断尿源性脓毒血症患者140例分为观察组,选择同期经皮肾镜或经输尿管镜手术后非尿源性脓毒症患者120例分为对照组.检测两组H2S、HBP、sTREM-1水平,分析H2S、HBP、sTREM-1在尿源性脓毒血症中的表达及对尿源性脓毒血症的诊断价值.结果观察组H2S、HBP、sTREM-1水平高于对照组(P<0.05).Logistic回归分析显示,H2S、HBP、sTREM-1水平升高是发生尿源性脓毒症的影响因素.ROC曲线显示,三项联合对尿源性脓毒症的诊断价值高于H2S、HBP、sTREM-1单项诊断(P<0.05).结论尿源性脓毒症患者H2S、HBP、sTREM-1水平明显升高,高水平的H2S、HBP、sTREM-1为尿源性脓毒症的风险预测分子.Objective To analyze the expression and correlation of H2S,HBP and sTREM-1 in urinary sepsis.Methods 140 patientswith urogenic sepsis diagnosed in our hospital from May 2019 to May 2021 were selected and divided into the research group.Patients with non-urogenic sepsis after percutaneous nephroscopy or ureteroscopy during the same period were selected.120 cases were selected into control group.The levels of H2S,HBP and sTREM-1 in the two groups were detected,and the expressions of H2S,HBP and sTREM-1 in urosepsis and their diagnostic value in urosepsis were analyzed.Results The levels of H2S,HBP and sTREM-1 in the study group were higher than those in the control group,with statistical significance(P<0.05).Logistic regression analysis showed that the elevated levels of H2S,HBP and sTREM-1 were the influencing factors of urosepsis.The ROC curve showed that the diagnostic value of the combination of the three items was higher than that of H2S,HBP,and sTREM-1 in the diagnosis of urosepsis,with statistical significance(P<0.05).Conclusion The levels of H2S,HBP,and sTREM-1 in patients with urosepsis are significantly increased,and high levels of H2S,HBP,and sTREM-1 are risk predictors of urosepsis.
