微生态制剂的序贯性肠内营养对重型颅脑损伤患者营养状况的影响

摘　　要：目的探讨添加微生态制剂序贯性肠内营养支持方案在重型颅脑损伤患者中的应用价值.方法选取2018年12月至2020年7月本院收治的重型颅脑损伤患者98例,按随机数字表法分为观察组与对照组,每组各49例.对照组实施序贯性肠内营养支持方案,观察组实施添加微生态制剂的序贯性肠内营养支持方案.比较两组患者营养状况、肠黏膜屏障指标变化及营养支持相关不良反应发生情况.结果相较于同期对照组,观察组营养支持第7天,血清总蛋白(TP)、白蛋白(ALB)、血红蛋白(Hb)水平明显较高(P<0.05);相较于同期对照组,观察组营养支持第7天,血清D-乳酸(D-LA)、内毒素(BT)、二胺氧化酶(DAO)水平下降更明显(P<0.05);观察组营养支持相关不良反应发生率(12.24%)与对照组(28.57%)比较,明显较低(P<0.05).结论添加微生态制剂的序贯性肠内营养支持方案能改善重型颅脑损伤患者营养状况及肠黏膜屏障功能,并能减少相关不良反应.Objective To explore the application value of sequential enteral nutrition support program supplemented with probiotics in patients with severe head injury.Methods From December 2018 to July 2020,98 patients with severe head injury were selected for treatment in our hospital and divided into groups by the random number table method,with 49 cases in each group.The control group implemented sequential enteral nutrition support program,and the observation group implemented sequential enteral nutrition support program supplemented with probiotics.The changes of nutritional status,the intestinal mucosal barrier indexes and the occurrence of adverse reactions related to nutritional support of two groups were compared and analyzed.Results Compared with the control group at the same period,the serum total protein(TP),albumin(ALB)and hemoglobin(Hb)levels at the 7th day of nutritional support in the observation group were significantly higher(P<0.05).Compared with the control group at the same period,the serum D-lactic acid(D-LA),endotoxin(BT),diamine oxidase(DAO)levels at the 7th day of nutritional support in the observation group were decreased more significantly(P<0.05).The incidence of nutritional support-related adverse reactions in the observation group(12.24%)was significantly lower compared with the control group(28.57%)(P<0.05).Conclusion The sequential nutritional support program with the addition of probiotics can improve the nutritional status and intestinal mucosal barrier function of patients with severe head injury,and can reduce related adverse reactions.It is worthy of promotion.
