中药外用治疗卒中后肩手综合征的临床研究

摘　　要：卒中后肩手综合征(Shoulder Hand Syndrome,SHS)是脑血管病后最常见并发症之一,SHS发病机制存在很大争议,而医师临床干预不同,导致每位患者预后差异很大,其发病率高,致残率高,目前临床上缺乏特效的治疗方法。中药外用是中医外治法主要疗法之一,其起效迅速、费用低、操作简便,疗效独特、安全,无不良反应。使用中药外用治疗卒中后肩手综合征疗效明显,相对于西医疗法具有操作简单、痛苦小、疗效稳定、预后佳、易接受、依从性高等优点。通过查阅近5年使用中药外治疗法治疗卒中SHS的相关文献资料,分别从中药熏洗、中药穴位贴敷、中药药罐法、中药熨敷、中药蜡熨、中药涂擦、中药刮痧、中药仪器导入以及综合疗法等方面予以归纳整理,以期为今后使用中药外用疗法治疗中风后肩手综合征的方案选取提供更为有力的依据,为临床提供参考。Shoulder hand syndrome(SHS)is one of the most common complications after cerebrovascular disease.The pathogenesis of SHS is very controversial.Different doctors have different clinical interventions,resulting in a great difference in the prognosis of each patient.Its incidence rate is high,and its disability rate is high.At present,there is no specific treatment in clinical practice.External use of traditional Chinese medicine is one of the main therapies of external treatment of traditional Chinese medicine.It has rapid onset,low cost,simple operation,unique efficacy,safety and no adverse reactions.The external use of traditional Chinese medicine in the treatment of post stroke shoulder hand syndrome has obvious effect.Compared with western medicine,it has the advantages of simple operation,less pain,stable effect,good prognosis,easy acceptance and high compliance.By reviewing the relevant literature on the treatment of stroke SHS with external treatment of traditional Chinese medicine in the past five years,though summarized and sorted out the following aspects respectively:fumigation and washing of traditional Chinese medicine,acupoint application of traditional Chinese medicine,cupping of traditional Chinese medicine,ironing of traditional Chinese medicine,wax ironing of traditional Chinese medicine,rubbing of traditional Chinese medicine,scraping of traditional Chinese medicine,introduction of traditional Chinese medicine instruments,and comprehensive treatment,so as to provide a more powerful basis for the selection of plans for the treatment of post stroke shoulder hand syndrome with external treatment of traditional Chinese medicine,with a view to providing reference for clinical practice.
