ANALYSIS OF IMPORTED MALARIA CASES AND VECTOR ANOPHELES POPULATION IN WUXI FROM 2011 TO 2020

摘  要:随着中国和非洲以及东南亚国家交流的日益增多,输入性疟疾疫情压力呈上升趋势。为做好无锡市输入性疟疾的防控工作,本文总结了无锡市2011-2020年输入性疟疾病例特征和媒介按蚊的种群特征,为输入性疟疾的精准防控提供科学依据。根据流行病个案调查信息,分析2011-2020年输入性疟疾病例的来源国别、职业构成、年龄结构、月份变化等特征,同时分析无锡市居民区、医院、公园、农户和猪圈环境的疟疾媒介按蚊的密度变化趋势等特征。结果显示,2011-2020年,无锡市共报告输入性疟疾病例91例,其中恶性疟68例;20~59岁病例数最多,占97.8%;技术工人和一般工人占比最高,合计占57.1%;1、6月份病例最多;病例来源国主要位于赤道附近。无锡市疟疾媒介按蚊为中华按蚊。居民区、医院、农户、牲畜棚的中华按蚊密度逐年显著下降,与年份序列的线性相关系数(r)分别为-0.7400(P<0.05)、-0.7116(P<0.05)、-0.7747(P<0.01)、-0.641(P<0.05)。猪圈中蚊虫密度最高,8月份前后分别呈指数型增长(R2=0.998,P<0.001)和降低(R2=0.9538,P<0.05)。结果表明,输入性疟疾是无锡市疟疾疫情再传播的主要风险来源,赴非务工人员是风险传播的主要人群。中华按蚊是无锡市疟疾再传播风险的主要媒介,农村是传播风险较高的环境类型。针对无锡市输入性疟疾病例特征和媒介特征,针对赴非人员和农村地区进行定向的疟疾防治科普宣传,将是疟疾精准防控的主要措施。With the increasing communications among China, Africa and Southeast Asian countries, the epidemic risk of imported malaria is also soaring. In order to enhance the prevention and control on imported malaria in Wuxi, the imported malaria cases and the vector mosquito populations in Wuxi from 2011 to 2020 were analyzed to provide scientific basis for the precise prevention and control. According to the epidemiological case survey information, the characteristics of the country of original sites, occupations, age structure, monthly changes, etc. of imported malaria cases from 2011 to 2020 were statistical analyzed. The trend in the density of Anopheles population in residential areas, hospitals, parks, farmers and pig pens in Wuxi was catalog. In the past 10 years, 91 imported malaria cases have been reported, including 68 cases of falciparum malaria. The number of cases aged 20-59 years was the largest, accounting for 97.8%. The infected proportion of skilled workers and ordinary workers is the highest, accounting for 57.1% in total. The most cases appeared in June;The original countries of the cases are mainly located near the Equator. Anopheles sinensis is the only valid vector of malaria in Wuxi city. The density of An. sinensis in residential areas, hospitals, farmers and livestock sheds decreased significantly by years, and the linear correlation coefficients(r) in the year series were-0.7400(P<0.05),-0.7116(P<0.05),-0.7747(P<0.01) and-0.641(P<0.05), respectively. Mosquito density in the pigsty was the highest, with exponential growth(R~2=0.998, P<0.001) and then decrease(R~2=0.9538, P<0.05) before and after August. Imported malaria is the main risk of malaria retransmission in Wuxi, and migrant workers for Africa are the main population of risk transmission. Rural areas are the typical habitat environment with high transmission risk.
