都昌县农村居民高血压患病及影响因素分析

摘　　要：目的掌握都昌县农村居民高血压的患病现况及其危险因素,为该地区高血压防控提供参考依据。方法采用整群抽样的方法,在都昌县24个乡镇中随机抽取1个乡镇,再从中根据村委会的居民稳定性、常住人口数量及工作人员能力抽取8个村委会,以村委会为单位对35~75岁居民开展面对面询问问卷调查、体格测量和实验室检测。结果该次研究共纳入调查对象3498名农村居民,女性占60.86%,男性占39.14%;平均年龄(59.70±9.70)岁。都昌县35~75岁农村居民高血压患病率为44.91%,标化率为32.47%;女性患病率为43.21%,男性为47.55%。无论是女性还是男性,高血压患病率均随年龄的增长而上升。二分类多因素非条件logistic回归分析提示,年龄增长[40~49岁组(OR=1.219,95%CI:0.673~2.210);50~59岁组(OR=2.903,95%CI:1.635~5.154);60~69岁组(OR=6.584,95%CI:3.673~11.801);70~75岁组(OR=9.339,95%CI:5.116~17.047)]、离异或丧偶(OR=1.347,95%CI:1.079~1.681)、超重或肥胖(OR=2.702,95%CI:2.313~3.157)、糖尿病(OR=1.792,95%CI:1.356~2.368)和血脂异常(OR=1.282,95%CI:1.040~1.579)是高血压患病的危险因素。结论江西省都昌县中老年农村居民高血压患病率较高,应重点关注和加强年龄大、婚姻状况不良、超重或肥胖、糖尿病和血脂异常人群的血压管理。Objective To explore the prevalence and risk factors of hypertension among rural residents in Duchang County of Jiangxi Province,so as to provide reference for the prevention and control of hypertension in this area.Methods One township was randomly chosen from 24 townships in Duchang County by the cluster sampling method,and then 8 village committees were selected according to the stability of residents,the number of permanent residents,and the working capacities of the staff.Face-to-face questionnaire surveys,physical measurements and laboratory tests were conducted,by village committee,among the residents aged 35-75 years.Results A total of 3498 rural residents were included in this study,with females accounting for 60.86%and males accounting for 39.14%.The average age was(59.70±9.70)years old.The prevalence of hypertension in rural residents aged 35-75 years in Duchang County was 44.91%,and the standardized rate was 32.47%.The prevalence was 43.21%in females and 47.55%in males.The prevalence of hypertension increased with age in both females and males.Binary multivariate unconditional Logistic regression analysis suggested that the risk factors for hypertension were age increase(group of 40-49 years old:OR=1.219,95%CI:0.673-2.210;group of 50-59 years old:OR=2.903,95%CI:1.635-5.154;group of 60-69 years old:OR=6.584,95%CI:3.673-11.801;group of 70-75 years old:OR=9.339,95%CI:5.116-17.047),divorced or widowed status(OR=1.347,95%CI:1.079-1.681),overweight or obesity(OR=2.702,95%CI:2.313-3.157),diabetes mellitus(OR=1.792,95%CI:1.356-2.368)and dyslipidemia(OR=1.282,95%CI:1.040~1.579).Conclusions The prevalence of hypertension among middle-aged and elderly rural residents in Duchang County of Jiangxi Province was high.The management of blood pressure should be focused on and strengthened in populations of older age,poor marital status,overweight or obesity,diabetes,and dyslipidemia.
