摘 要:目的 观察本体感觉神经肌肉促进(PNF)技术联合功能性电刺激踏车(FES-cycling)治疗对脑卒中患者上肢功能的影响。方法 选取2021年5月至2022年5月在该院康复科收治的60例脑卒中患者作为研究对象,采用随机数字表法分为PNF组(A组)、FES-cycling组(B组)和PNF+FES-cycling组(C组),每组20例,共持续治疗4周。对3组患者治疗前后、随访时(治疗结束后2周)上肢肌张力[改良Ashworth分级(MAS)]及运动功能[Fugl-Meyer运动功能评定量表(FMA)]、日常生活活动能力[ADL,改良Barthel指数(MBI)]进行评定。结果 3组患者治疗后、随访时MAS评分均较治疗前降低,且C组患者均低于A、B组,FMA、MBI评分均较治疗前提高,且C组患者均高于A、B组,差异均有统计学意义(P<0.05)。结论 PNF技术联合FES-cycling治疗可显著改善脑卒中患者上肢痉挛情况,提高肢体运动功能及ADL。Objective To observe the effect of proprioceptive neuromuscular promotion(PNF) technology combined with functional electrical stimulation treadmill(FES-cycling) on upper limb function in stroke patients.Methods A total of 60 stroke patients receiving treatment in the Rehabilitation Department of this hospital were selected and divided into PNF group(group A),FES-cycling group(group B) and PNF combined with FES-cycling group(group C) according to random number table method, with 20 cases in each group.The patients were treated for four weeks.The muscle tone, motor function and activities of daily living(ADL) of the upper limbs of the three groups were evaluated before treatment, after treatment and during follow-up.Results After treatment and at follow-up, the scores of modified Ashworth scale(MAS) in the three groups was significantly lower than those before treatment, and those in group C were significantly lower than those in group A and B,with statistical significance(P<0.05).The scores of Fugl-Meyer motor function rating scale(FMA) and Modified Barbi index(MBI) in the three groups were significantly higher than those before treatment, and the scores of FMA and MBI in the group C were significantly higher than those in the group A and group B,with statistical significance(P<0.05).Conclusion PNF combined with FES-cycling can significantly improve the spasticity of the upper limb in stroke patients and improve the limb motor function and ADL.
