补肾通络畅膜方联合雌孕激素治疗中度宫腔粘连分离术后再粘连的临床疗效

摘　　要：目的研究补肾通络畅膜方联合雌孕激素防治中度宫腔粘连分离术后再粘连的临床疗效。方法选取60例宫腔镜确诊为中度宫腔粘连患者,根据随机数字表法分为对照组与治疗组,每组30例。两组患者均行宫腔镜下宫腔粘连分离术(TCRA),对照组术后予以雌孕激素治疗,治疗组在对照组基础上加用补肾通络畅膜方治疗,两组均治疗3个周期。比较两组患者术后宫腔粘连评分、中医证候评价疗效、D-二聚体指标变化。结果两组患者治疗后宫腔粘连评分降低,与治疗前比较,差异有统计学意义(P<0.05);与对照组比较,治疗组患者治疗后宫腔粘连评分降低更明显,差异有统计学意义(P<0.05)。治疗后,对照组总有效率为73.3%,治疗组为83.3%,比较差异无统计学意义(P>0.05)。两组患者治疗后D-二聚体值均较治疗前降低,差异有统计学意义(P<0.05);与对照组比较,治疗组治疗后D-二聚体值降低但差异无统计学意义(P>0.05)。结论补肾通络畅膜方联合雌孕激素对中度宫腔粘连分离术后患者能改善患者临床症状,更好地恢复宫腔形态,降低宫腔粘连评分,降低血清D-二聚体水平,从而有效防治术后再粘连。Objective To investigate the effect of Bushen Tonglu Changmo Formula Combined with estrogenic and progesterone hormone for the treatment of post-operative adhesion after moderate intrauterine adhesion(IUA)separation.Methods 60 patients diagnosed with moderate intrauterine adhesions by hysteroscopy were selected and divided into control group and treatment group according to random number table method,with 30 cases in each group.Patients in both groups received hysteroscopic intrauterine adhesion separation(TCRA),the control group was treated with estrogen after surgery,and the treatment group was additionally treated with Bushen Tongluo Chang membrane prescription on the basis of the control group.Both groups were treated for 3 cycles.Postoperative intrauterine adhesion score,TCM symptom evaluation effect and D-dimer index changes were compared between the two groups.Results The intrauterine adhesion score of the two groups decreased after treatment,and the difference was statistically significant compared with before treatment(P<0.05).Compared with the control group,the intrauterine adhesion score in the treatment group decreased more significantly after treatment,and the difference was statistically significant(P<0.05).After treatment,the total effective rate was 73.3%in the control group and 83.3%in the treatment group,with no statistical significance(P>0.05).The D-dimer value of 2 groups after treatment was lower than that before treatment,and the difference was statistically significant(P<0.05).Compared with the control group,there was no statistical significance in the reduction of D-dimer value in the treatment group after treatment(P>0.05).Conclusion Bushen Tonglu Changmo Formula Combined with estrogenic and progesterone hormone can effectively improve the clinical symptoms,decrease IUA scores and plasma D-dimer level of patients with post-operative adhesion after moderate intrauterine adhesion separation.The treatment can effectively prevent adhesion after TCRA.
