不孕症妇女子宫动脉血流测定异常药物干预治疗后妊娠结局的研究

摘　　要：目的研究不孕症妇女测定子宫动脉血流异常经药物干预治疗后的妊娠结局。方法选取60例不孕症妇女,经阴道彩色多普勒超声检测卵泡期与黄体期的子宫动脉血流阻力指数高及内膜下的血流灌注差。将患者根据药物治疗不同分为A组、B组、C组,每组20例。A组给予西地那非联合补肾化瘀汤治疗,B组给予西地那非治疗,C组给予阿司匹林治疗。每组根据卵泡期用药后Applehaum评分变化,比较三组子宫动脉血流阻力指数及妊娠结局。结果治疗第1个月经周期、第2个月经周期、第3个月经周期、孕12周时,A组子宫动脉血流阻力指数下降均大于B组、C组,差异有统计学意义(P<0.05);B组卵泡期子宫动脉血流阻力指数下降大于C组,差异有统计学意义(P<0.05)。A组治疗后妊娠结局优于B组、C组,差异有统计学意义(P<0.05)。结论对子宫动脉血流异常的不孕症妇女治疗,采用西地那非联合补肾化瘀汤效果明显,妊娠率高。Objective To study the pregnancy outcome of abnonnal uterine artery blood flow in infertile women after drug intervention.Methods 60 infertile women were selected to detect the high uterine artery resistance index and the poor subintimal perfusion in the follicular and luteal phases by transvaginal color Doppler ultrasound.The patients were divided into group A,group B and group C according to different drug treatments,with 20 cases in each group.Group A was given sildenafil combined with Bushenhuayu decoction,group B was given sildenafil and group C was given aspirin.According to the change of Applehaum score after treatment at follicular stage,uterine artery blood resistance index and pregnancy outcome were compared among the three groups.Results At the first menstrual cycle,the second menstrual cycle,the third menstrual cycle and 12 weeks of pregnancy,the decrease of uterine artery blood resistance index in group A was greater than that in group B and group C,and the difference was statistically significant(P<0.05).The decrease of uterine artery flow resistance index in group B was greater than that in group C,and the difference was statistically significant(P<0,05).The pregnancy outcome of group A after treatment was better than group B and group C,and the difference was statistically significant(P<0.05).Conclusion Sildenafil combined with Bushenhuayu decoction has obvious effect on the treatment of infertile women with abnormal uterine artery blood flow,and the pregnancy rate is high.
