摘  要:目的评估唾液酸酶法、免疫荧光法应用在阴道加德纳菌(GV)检测中的效果。方法选取妇科门诊就诊的115例已婚妇女,采集阴道分泌物标本,且以唾液酸酶法、免疫荧光法检测,参照Amsel法诊断结果,评价不同检查方法诊断效能及不同年龄段检测结果。结果115例已婚妇女经Amsel法确诊43例GV阳性,唾液酸酶法检测GV时的准确率(83.48%)、敏感度(74.42%)、阴性预测值(NPV)(85.33%)低于免疫荧光法(93.91%)、(93.02%)、(95.77%),差异有统计学意义(P<0.05),唾液酸酶法的特异度(88.89%)、阳性预测值(PPV)(80.00%)与免疫荧光法(94.44%)、(90.91%)比较,差异无统计学意义(P>0.05)。Amsel法、唾液酸酶法、免疫荧光法在不同年龄段的阳性检出率比较,差异无统计学意义(P>0.05)。结论在GV检测中采用免疫荧光法准确率、敏感度、NPV较高,具有临床推广价值。Objective To evaluate the effects of sialase and immunofluorescence in the detection of Gardnerella vaginalis(GV).Methods Vaginal secretions were collected from 115 married women in gynecological outpatient department.Sialase method and immunofluorescence method were used to detect vaginal secretions.The diagnostic efficacy of different methods and the results of different age groups were evaluated according to the diagnostic results of Amsel method.Results Among 115 married women,43 cases were confirmed positive for GV by Amsel method.The accuracy(83.48%),sensitivity(74.42%)and negative predictive value(85.33%)of GV detected by sialase method were lower than those detected by immunofluorescence method(93.91%),(93.02%)and(95.77%).The difference was statistically significant(P<0.05),but the specificity(8&89%)and positive predictive value(PPV)(80.00%)of sialase method were not statistically significant compared with immunofluorescence method(94.44%)and immunofluorescence method(90.91%)(P>0.05).There was no significant difference in the positive detection rate of Amsel method,sialase method and immunofluorescence method in different age groups(P>0.05).Conclusion In the detection of GV,immunofluorescence method has high accuracy,sensitivity and NPV which has the value of clinical popularization.
