摘  要:目的探究活动性结核病患者血清高迁移率族蛋白B1(high mobility group box 1 protein,HMGB1)、中性粒细胞/淋巴细胞(neutrophil-to-lymphocyte,NLR)的变化,了解其与患者细胞免疫功能水平及38000蛋白的相关性。方法将2019年1月至2021年6月吉林市结核病防治研究所收治的60例活动性结核病患者设为A组,将同期收治的60例非活动性结核病患者设为B组,另将60名健康志愿者设为C组,比较3组患者HMGB1表达水平、NLR、细胞免疫功能相关指标(CD3^(+)、CD4^(+)、CD8^(+)和CD4^(+)/CD8^(+)T淋巴细胞)和38000蛋白的差异,分析HMGB1、NLR与活动性结核病患者细胞免疫功能水平和38000蛋白的相关性。结果A、B、C组患者HMGB1分别为(5.63±1.40)ng/mL、(4.00±1.59)ng/mL和(2.19±0.88)ng/mL,三组间比较差异有统计学意义(F_(HMGB1)=101.72,P<0.001);NLR分别为(3.17±0.41)、(1.88±1.51)和(1.43±0.11),三组间比较差异有统计学意义(FNLR=727.79,P<0.001)。CD3^(+)、CD4^(+)和CD4^(+)/CD8^(+)细胞水平与HMGB1(r=-0.66、-0.66和-0.70,P均<0.001)和NLR(r=-0.88、-0.88和-0.86,P<0.001)呈负相关,CD8^(+)、38000蛋白与HMGB1(r=0.67,0.40,P均<0.001)和NLR(r=0.80,0.61,P<0.001)均呈正相关。结论活动性结核病患者HMGB1、NLR水平升高,并且与细胞免疫功能指标及38000蛋白具有相关性,可将其用于评估患者的免疫能力和感染程度,为治疗提供指导依据。Objective To explore the changes of serum high mobility group box 1 protein(HMGB1)and neutrophil to lymphocyte ratio(NLR)in patients with active tuberculosis,and the correlation between HMGB1/NLR and cellular immune function/38000 protein.Methods A total of 60 patients with active tuberculosis admitted in Research Institute of Tuberculosis Prevention and Treatment of Jilin from January 2019 to June 2021 were enrolled as group A.During the same period,60 patients with non-active tuberculosis and 60 healthy people were enrolled as group B and group C,respectively.The differences of HMGB1,NLR levels,cellular immune function related indicators(CD3^(+),CD4^(+),CD8^(+),CD4^(+)/CD8^(+)T lymphocytes)and 38000 protein levels in 3 groups were compared.The correlation between HMGB1/NLR and cellular immune function/38000 protein was analyzed.Results In group A,B and C,the levels of HMGB1 were(5.63±1.40)ng/mL,(4.00±1.59)ng/mL and(2.19±0.88)ng/mL,respectively,and there was significantly different among these groups(F_(HMGB1)=101.72,P<0.001).The levels of NLR were(3.17±0.41),(1.88±1.51)and(1.43±0.11),respectively,and there was significantly different among these groups(FNLR=727.79,P<0.001).Levels of CD3^(+),CD4^(+)and CD4^(+)/CD8^(+)were negatively related with HMGB1(r=-0.66,-0.66 and-0.70,P all<0.001)and NLR(r=-0.88,-0.88 and-0.86,P all<0.001).Levels of CD8^(+)and 38000 protein were positively correlated with HMGB1(r=0.67 and 0.40,P both<0.001)and NLR(r=0.80 and 0.61,P both<0.001).Conclusions The levels of HMGB1 and NLR increase in patients with active tuberculosis,and are related with the indexes of cellular immune function and 38000 protein.HMGB1 and NLR can be used to evaluate the immune capacity and infection degree of patients and provide guidance for treatment.
