MRCP联合小视野高分辨DWI对壶腹周围癌的检出价值

摘　　要：目的探讨磁共振胰胆管造影(MRCP)联合小视野高分辨率扩散加权成像(DWI)对壶腹周围癌的检出价值。方法回顾性分析经病理证实的26例壶腹周围癌患者的MRCP、壶腹周围区域的单次激发平面回波成像(SS-EPI)DWI(c-DWI)和二维空间选择性射频激励脉冲放大EPI-DWI(z-DWI)图像,对比分析MRCP联合c-DWI组和MRCP联合z-DWI组对壶腹周围癌的检出能力。结果MRCP联合z-DWI组对壶腹周围癌的检出能力(4.35±0.80)要高于MRCP联合c-DWI组(3.77±1.14)(P<0.05)。对于直径不超过2 cm的壶腹周围癌(15例),MRCP联合z-DWI组的检出能力(4.33±0.82)亦高于MRCP联合c-DWI组(3.47±1.25)(P<0.05)。结论MRCP联合小视野高分辨率DWI有助于提高壶腹周围癌病灶的检出能力。Objective To investigate detection value of MRCP combined with reduced field of view high resolution DWI for periampullary carcinomas.Methods Twenty-six patients with periampullary carcinomas confirmed by pathology were analyzed in the study in which MRCP,single-shot echo-planar imaging(ss EPI)DWI(c-DWI)and two-dimensional dynamic spatially selective RF excitation pulses zoomed echo-planar imaging DWI(z-DWI)were performed.Detection accuracy scores of the combined image sets of MRCP and z-DWI were compared with that of combined set of MRCP and c-DWI.Results Detection accuracy scores of the combined set of MRCP and z-DWI(4.35±0.80)were significantly higher compared to those of the MRCP and c-EPI DWI set(3.77±1.14)(P<0.05).For periampullary lesions with diameter less than or equal to 2 cm(n=15),detection accuracy scores of the combined set of MRCP and z-DWI(4.33±0.82)were also significantly higher compared to those of the MRCP and c-EPI DWI set(3.47±1.25)(P<0.05).Conclusion MRCP combined with reduced field of view high resolution DWI was helpful in the increased ability of detection accuracy for the periampullary carcinomas.
