镍钛记忆合金环抱器切开复位内固定术与非手术治疗外伤性多发肋骨骨折的临床对比

摘　　要：目的比较镍钛记忆合金环抱器切开复位内固定术与非手术治疗外伤性多发肋骨骨折(MRF)的临床效果。方法选取2017年4月-2020年12月确诊为MRF患者92例,42例接受镍钛记忆合金环抱器切开复位内固定术治疗的患者纳入手术组,50例仅接受保守治疗的患者纳入保守组。比较两组骨折愈合情况、康复进度、VAS评分、肺功能和并发症情况。结果手术组肋骨骨折愈合情况明显优于保守组(P<0.05)。手术组下床时间、镇痛时间、住院时间均明显比保守组短(P<0.05)。两组组内VAS评分比较,治疗时间越长,VAS评分越低(P<0.05);两组治疗前VAS评分无显著差异(P>0.05),手术组治疗3d、7d、14d的VAS评分均明显比保守组低(P<0.05)。两组并发症发生率差异不显著(P>0.05)。结论镍钛记忆合金环抱器切开复位内固定术治疗外伤性MRF效果优于非手术治疗,能改善骨折愈合,加快康复进程,减轻疼痛,减少肺功能减退。Objective To compare the clinical effect of open reduction and internal fixation with Nitinol embracing device and non-surgical treatment of traumatic multiple rib fractures(MRF).Methods A total of 92 patients diagnosed with MRF from April 2017 to December 2020 were selected,42 patients who received nickel-titanium memory alloy embracing open reduction and internal fixation were included in the surgical group,and 50 patients who only received conservative treatment were included in the conservative group.The fracture healing,rehabilitation progress,VAS score,pulmonary function and complications were compared between the two groups.Results The rib fracture healing in the operation group was significantly better than that in the conservative group(P<0.05).The time to get out of bed,analgesia,and hospital stay in the operation group were significantly shorter than those in the conservative group(P<0.05).Compared with the VAS score in the two groups,the longer the treatment time,the lower the VAS score(P<0.05);there was no significant difference in VAS score between the two groups before treatment(P>0.05);the VAS score of the operation group was significantly lower than that of the conservative group at 3d,7d and 14d after treatment(P<0.05).Significantly lower than the conservative group(P<0.05).There was no significant difference in the incidence of complications between the two groups(P>0.05).Conclusion The effect of open reduction and internal fixation with nickel-titanium memory alloy embracing device in the treatment of traumatic MRF is better than that of non-surgical treatment,which can improve fracture healing,speed up the recovery process,reduce pain,and reduce lung function decline.
