基于ERAS理念的个体化护理应用于肝胆外科围术期中的效果

摘　　要：目的探讨基于快速康复(ERAS)理念的个体化护理应用于肝胆外科围术期中的效果。方法按照随机数字表法将本院2018年1月-2021年1月收治的81例肝胆外科患者分为两组,常规组40例给予常规外科护理,ERAS组41例给予基于ERAS理念的个体化护理。观察两组疼痛、胃肠功能、负性情绪、生活质量及并发症发生情况。结果ERAS组术后VAS评分12h、24h、72h均低于常规组(P<0.05);ERAS组首次肛门排气时间、进食时间、排便时间均短于常规组(P<0.05);出院时,ERAS组SDS评分、SAS评分均低于常规组(P<0.05);入院时,ERAS组躯体功能、物质生活、心理功能、社会功能均高于常规组(P<0.05);ERAS组并发症发生率低于常规组(P<0.05)。结论基于ERAS理念的个体化护理应用于肝胆外科围术期,可有效减轻患者疼痛程度,促进胃肠功能恢复,改善负性情绪,提高生活质量,且并发症发生率低。Objective To explore the effect of individualized nursing based on the concept of rapid rehabilitation(ERAS)in the perioperative period of hepatobiliary surgery.Methods According to the random number table method,81 patients with hepatobiliary surgery treated in our hospital from January 2018 to January 2021 were divided into two groups.40 patients in the routine group were given routine surgical nursing,and 41 patients in the eras group were given individualized nursing based on the concept of eras.The pain,gastrointestinal function,negative emotion,quality of life and complications were observed.Results The VAS scores of eras group at 12h,24h and 72h were lower than those of routine group(P<0.05);The first anal exhaust time,eating time and defecation time in eras group were shorter than those in routine group,and(P<0.05);At discharge,SDS score and SAS score in eras group were lower than those in routine group and(P<0.05);At admission,the scores of physical function,material life,psychological function and social function in eras group were higher than those in routine group(P<0.05);The incidence of complications in eras group was lower than that in routine group(P<0.05).Conclusion The application of individualized nursing based on eras concept in the perioperative period of hepatobiliary surgery can effectively reduce the degree of pain,promote the recovery of gastrointestinal function,improve negative emotion,improve the quality of life,and the incidence of complications is low.
