参附注射液联合西药治疗慢性充血性心力衰竭的Meta分析

摘　　要：目的:系统评价参附注射液联合西药常规治疗慢性充血性心力衰竭的临床疗效,为临床提供参考依据。方法:计算机检索中国知网(CNKI)、万方数据库(Wanfang database)、维普数据库(VIP)、中国生物医学文摘数据库(CBM)、Pubmed Centeral、荷兰医学文摘数据库(Embase)、Cochrane图书馆等中英文数据库,检索全部相关临床随机对照试验(RCT),经过文献筛选和评价质量后提取有效数据,基于RevMan5.3软件进行Meta分析。结果:最终纳入16篇文献,总计1402例患者。Meta分析结果显示,参附注射液联合西药常规治疗慢性充血性心力衰竭在提高临床综合疗效[RR=1.20,95%CI(1.13,1.28),P<0.00001],左室射血分数[MD=2.13,95%CI(1.39,2.87),P<0.0001],每搏搏出量[MD=3.17,95%CI(1.88,4.46),P<0.00001],6min步行距离试验[MD=87.72,95%CI(54.80,120.64),P<0.00001]和降低左心室舒张末期内径[MD=-3.60,95%CI(-5.22,-1.97),P<0.0001],B型利钠肽[MD=-30.22,95%CI(-43.59,-16.86),P<0.00001]等数值指标上均优于单独使用西药常规治疗,其差异均具有统计学意义(P<0.05)。结论:联合参附注射液治疗慢性充血性心力衰竭的临床疗效优于单独使用西药。Objective:To systematically evaluate the clinical efficacy of Shenfu injection combined with western medicine for chronic congestive heart failure and provide evidence for clinical reference. Methods:Retrieved from CNKI,Wanfang, VIP,CBM,Pubmed Centeral, Embase, Cochrane Library and other Chinese and English databases, all RCTs about Shenfu injection in the treatment of chronic congestive heart failure were searched. After literature screening and quality evaluation, valid data were extracted for Meta-analysis using RevMan5.3 software. Results:Sixteen articles were included, with a total of 1402 patients. Meta-analysis results showed that Shenfu injection combined with western medicine in the conventional treatment of chronic congestive heart failure improved the comprehensive clinical efficacy [RR=1.20,95%CI(1.13,1.28),P<0.00001],LVEF[MD=2.13,95%CI(1.39,2.87),P<0.0001],SV[MD=3.17,95%CI(1.88,4.46),P<0.00001],6WMT[MD=87.72,95%CI(54.80,120.64),P<0.00001] and reduced LVEDD [MD=-3.60,95%CI(-5.22,-1.97),P<0.0001],BNP[MD=-30.22,95%CI(-43.59,-16.86),P<0.00001] and other numerical indicators were better than conventional treatment with western medicine alone, the differences were statistically significant(P<0. 05). Conclusion:Based on the conventional treatment of western medicine, the clinical efficacy of Shenfu injection in the treatment of chronic congestive heart failure is better than that of western medicine alone.
