基于中医古籍探析玄府理论源流及其指导下的中风病论治

摘　　要：“玄府”一词源于《黄帝内经》,金元医家刘完素创建玄府理论,阐明玄府体用、病机与治法,并首次应用于中风病论治,提出中风病“热郁玄府”实证病机,创通腑开玄、辛散开治法。明清时期,玄府理论得到发展,首提因虚玄阖病机及补虚助运开玄治法,并完善气血郁结、风痰上扰实证病机与对应治法。文章通过整理玄府古籍源流,探讨中风病急性早期始发态病机,总结中风病玄府病机在于通利开阖失调,病理因素为热、滞、痰、瘀、虚,治则以调和玄府为要,治法贵开忌阖,组方用药重在开通玄府,以期为中风病急性早期临床治疗提供思路。The word‘Xuanfu’originates from Huangdi Neijing.LIU Wan-su,a doctor in the Jin-Yuan dynasties,created the theory of Xuanfu,enriched the physical function and pathogenesis of Xuanfu,and applied it to the treatment of stroke for the first time.He proposed the empirical pathogenesis of‘heat stagnation in Xuanfu’of stroke,and created the treatment methods of‘Tongfu Kaixuan’and‘Xinsan Kaixuan’.In the Ming and Qing dynasties,the theory of Xuanfu was developed.It first proposed the pathogenesis of‘Yinxu Xuanhe’and the treatment of‘Buxu Zhuyun Kaixuan’,and improved the pathogenesis and corresponding treatment of qi and blood stagnation and wind and phlegm disturbance.This paper discusses the pathogenesis of acute early onset of stroke through sorting out the origin of Xuanfu theory,and summarizes that the pathogenesis of stroke is the imbalance of opening and closing,and the pathological factors are heat,stagnation,phlegm,blood stasis,and deficiency.The principle of treatment is to harmonize Xuanfu,and the treatment is more important than closing.The prescription focuses on opening and closing,so as to provide ideas for the ultra early local clinical treatment of stroke.
