摘　　要：目的探讨血液透析室医院感染控制的管理研究。方法将2020年5月-2021年5月在本院血液透析室行血液透析(HD)的84例患者随机分为2组。对照组42例行常规医院感染控制管理,观察组42例在纠正贫血、营养不良基础上联合预防医院感染措施,记录医院感染率,观察管理前后患者营养学及贫血指标,同时对管理效果进行评价,对透析液及反渗水进行细菌培养及内毒素检测。结果观察组医院感染率显著低于对照组(P<0.05)。管理前,两组白蛋白(ALB)、前白蛋白(PA)、转铁蛋白(TRF)、血红蛋白(Hb)、红细胞计数(RBC)水平比较,差异无统计学意义(P>0.05);管理后,观察组PA、TRF、Hb、RBC指标增高(P<0.05),对照组变化不明显(P>0.05)。观察组医院感染宣传、安全防护、卫生消毒及定期护理检测评分均显著高于对照组(P<0.05)。观察组透析液和反渗水的细菌培养及内毒素均显著低于对照组(P<0.05)。结论血液透析室管理中,在纠正贫血及营养不良基础上联合预防医院感染措施,可降低医院感染率,改善HD患者营养状况,纠正贫血,降低透析液和反渗水中微生物数量,管理效果理想。Objective To study management of nosocomial infection control in hemodialysis room.Methods The paper chose 84 patients with hemodialysis(HD)in hemodialysis room of our hospital from May 2020 to May 2021,and divided them into two groups randomly.42 cases in control group were treated with routine hospital infection control and management,42 cases in observation group with joint measures to prevent hospital infection on the basis of correcting anemia and malnutrition.Nosocomial infection rate,nutrition and anemia indicators before and after management was recorded and observed,management effect was evaluated,dialysate and reverse osmosis water were tested for bacterial culture and endotoxin.Results Nosocomial infection rate in observation group was significantly lower than control group(P<0.05).Before management,levels of albumin(ALB),prealbumin(PA),transferrin(TRF),hemoglobin(Hb)and red blood cell count(RBC)between two groups had no significant difference(P>0.05).After management,indexes of PA,TRF,Hb and RBC in observation group increased(P<0.05),while those of control group showed not change significantly(P>0.05).Scores of hospital infection propaganda,safety protection,sanitary disinfection and regular nursing examination in observation group was significantly higher than control group(P<0.05).Bacterial culture and endotoxin of dialysate and reverse osmosis water in observation group was significantly lower than control group(P<0.05).Conclusion During management of hemodialysis room,on the basis of correcting anemia and malnutrition,joint measures can reduce hospital infection rate,improve nutritional status of HD patients,correct anemia,reduce number of microorganisms in dialysate and reverse osmosis water,with ideal management effect.
