摘　　要：目的 探讨基于个案管理模式的随访对空巢老年糖尿病患者生活质量的影响。方法选取2020年6月—2021年5月在医院住院的空巢老年糖尿病患者82例为研究对象,按照组间基本资料具有可比性的原则分为观察组42例和对照组40例。对照组实施常规教育,观察组在对照组基础上接受个案管理模式。干预前、干预后12个月收集患者的糖化血红蛋白、血脂指标、生存质量、自我管理能力的结果并进行比较。结果 干预后,观察组糖化血红蛋白、血脂指标均低于对照组,差异有统计学意义(P<0.05);干预后,观察组的生存质量各维度评分均低于对照组,差异有统计学意义(P<0.05);干预后,观察组自我管理能力各维度评分及总分均高于对照组,组间比较差异有统计学意义(P<0.05)。结论 基于个案管理模式的随访能提高空巢老年糖尿病患者的生活质量和自我管理能力,且能改善患者的糖化血红蛋白及血脂指标。Objective To explore the impact of follow-up based on case management model on the quality of life of empty nest elderly diabetes patients.Methods A total of 82 empty nest elderly diabetes patients hospitalized in the hospital from June 2020 to May 2021 were selected as the study subjects.According to the principle of comparability of basic data between groups,they were divided into an observation group of 42 cases and a control group of 40 cases.The control group received routine education,while the observation group received case management mode on the basis of the control group.The results of glycosylated hemoglobin,blood lipid index,quality of life and self-management ability of patients were collected and compared before and 12 months after intervention.Results After intervention,the glycosylated hemoglobin and blood lipid index in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).After intervention,the each dimension scores of life quality in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).After intervention,the each dimension scores and total score of self-management ability in the observation group were higher than those in the control group,and the differences between the groups were statistically significant(P<0.05).Conclusion Follow-up based on case management mode can improve the quality of life and self-management ability of empty nest elderly diabetes patients,and can improve the glyco sylated hemoglobin and blood lipid indicators of patients.
