摘  要:目的:探究抑郁症(Major Depressive Disorder,MDD)患者对负性刺激的转换功能是否受损及其相关神经电生理特征。方法:采集37例首发未服药抑郁症患者和39例对照组(Healthy Controls,HC)被试在完成情绪任务转换范式时的行为学及脑电数据。取P2、N2、P3和PSP为目标成分,采用2(组别:MDD/HC)×2(任务:重复任务/转换任务)×4(电极:FZ/FCZ/CZ/PZ)重复测量方差分析比较各成分波幅和潜伏期的组间差异。结果:(1)MDD组在转换任务中的N2波幅显著小于重复任务,而在HC组中差异不显著,且MDD组的N2差异波波幅显著小于HC组;(2)MDD组在转换任务中的P3波幅显著大于重复任务,而在HC组中差异不显著,且MDD组P3差异波波幅显著大于HC组;(3)MDD组的PSP波幅显著大于HC组。结论:抑郁症患者对负性刺激存在转换功能受损,反映早期冲突抑制阶段和后期任务准备阶段的脑电成分异常可能是抑郁症患者转换功能受损的电生理基础。Objective: To investigate whether the shifting function was impaired in patients with Major Depressive Disorder(MDD) in response to negative stimuli, and to explore the relevant electrophysiological characteristics. Methods: A total of 37 first-episode drug-na?ve MDD patients and 39 matched healthy controls(HC) were included. Behavioral and electroencephalogram data was recorded during subjects performing emotional task switching paradigm. The amplitudes and latencies of Event-Related Potential components(i.e. P2, N2, P3, and PSP) were analyzed by 2(group: MDD/HC) ×2(task: repeat task/switch task) ×4(electrode: FZ/FCZ/CZ/PZ) repeated-measures ANOVA. Results:(1) In MDD group, the N2 amplitude in switch task was significantly smaller than that in repeat task, while which did not exist in HC group. And the amplitude of N2 difference wave in MDD group was smaller than that in HC group.(2) The P3 amplitude in switch task was significantly larger than that in repeat task within MDD group but not within HC group. MDD group showed enhanced amplitude of P3 difference wave compared with HC group.(3) MDD group exhibited higher PSP amplitude than HC group. Conclusion: The shifting function of negative stimuli was impaired in MDD patients, and the abnormal ERPs during the early conflicts inhibiting stage and the late preparation stage of the task may underlie the impaired shifting function of negative stimuli in MDD.
