超声测量健康志愿者掌指关节软骨的方法及影响因素研究

摘  要:目的探讨超声测量健康志愿者掌指关节(MCP)软骨的方法和影响因素。方法2021年11月至2022年4月在四川大学华西医院共招募95名健康志愿者为研究对象,采用超声测量其双手掌侧及背侧MCP软骨的厚度、可显示的长度和面积。随机抽取20名健康志愿者进行一致性评估,比较健康志愿者MCP软骨在背侧-纵断面、背侧-横断面、掌侧-纵断面及掌侧-横断面测量的差异,并与人口学特征参数进行相关性分析。结果超声测量健康志愿者MCP软骨厚度、可显示的长度和面积在不同检查者之间、同一检查者内部均具有良好的一致性(ICC_(不同检查者)=0.796~0.869,ICC_(同一检查者)=0.828~0.916)。在纳入的健康志愿者中,MCP软骨厚度、可显示的长度和面积在不同手指差异有统计学意义(P<0.05);掌侧MCP软骨较背侧更厚(P<0.05)。背侧-纵断面较背侧-横断面可显示的MCP软骨长度更长、面积更大(P<0.05);男性MCP软骨较女性更厚,可显示的长度更长、面积更大(P<0.05)。结论超声检查可显示健康志愿者MCP软骨,其背侧-纵断面为最佳测量位点;性别、手指与测量断面是超声测量健康志愿者MCP软骨的影响因素。Objective To explore the methods of ultrasonic measurement of metacarpophalangeal joint(MCP) cartilage in healthy people and the influencing factors. Methods A total of 95 healthy volunteers in West China Hospital of Sichuan University were recruited from November 2021 to April 2022. The thickness,displayable length and area of their MCP cartilage at the palmar and dorsal sides of both hands were measured by ultrasonography.Twenty of the recruited healthy volunteers were randomly selected for consistency evaluation. The differences of all the measured values of the MCP cartilages in the dorsal longitudinal,dorsal transverse,palmar longitudinal and palmar transverse sections were compared and analyzed for the correlation with the parameters of demographic characteristics. Results The thickness,displayable length and area of MCP cartilages measured by ultrasonography were of good consistency among different subjects and within the same subject,and the interclass correlation coefficient between subjects and within subjects were(0.796-0.869) and(0.828-0.916),respectively(P< 0.05). The measured values of the thickness,displayable length and area of MCP cartilages in different fingers were statistically different(P< 0.05). The thickness of the MCP cartilages at the palmar side were significantly greater than that at the dorsal side(P< 0.05). The displayable length and area of MCP cartilages in the dorsal longitudinal section were significantly greater than those in the dorsal transverse section(P< 0.05). The measured values of the thickness,displayable length and area of MCP cartilages in male subjects were significantly greater than those in female ones(P< 0.05). Conclusion Ultrasonography is effective in displaying the MCP cartilages of healthy volunteers. The dorsal longitudinal section is the best measuring site. Gender,finger and section are the influencing factors of the values of MCP cartilages in ultrasonic measurement.
