早发2型糖尿病周围神经病变血栓弹力图变化及危险因素分析

摘　　要：目的:分析早发2型糖尿病(T2DM)周围神经病变(DPN)患者血栓弹力图变化及危险因素分析。方法:选取2022-01-2022-06本院早发T2DM患者90例作为研究对象,根据是否合并DPN分为两组:DPN组(n=10例)和单纯T2DM组(n=80例),比较两组患者血小板参数、血凝五项和血栓弹力图指标的差异;Logistic回归分析T2DM合并DPN的危险因素。结果:DPN组凝固角(Angle角)为73.50(66.95-75.63)°高于单纯T2DM组67.10(63.50-68.50)°(P<0.05)。DPN组凝血块最大强度(MA值)为67.70(63.08-74.88)mm,高于单纯T2DM组59.50(54.95-61.70)mm(P<0.05)。Logistic回归分析结果显示,Angle角、MA值为DPN的独立危险因素(OR=0.618、0.683,P<0.05)。结论:早发T2DM合并DPN患者Angle角、MA值增大,二者均为DPN的独立危险因素,或可成为DPN诊断的血清学标记物。Objective:To analyze the changes and risk factors of thromboelastography in patients with early onset type 2 diabetes mellitus(T2DM)and peripheral neuropathy(DPN).Method:From January 2022 to June 2022,90 patients with early onset(T2DM)in our hospital were selected as the research object.According to whether or not they had DPN,they were divided into two groups:DPN group(n=10cases)and simple T2DM group(n=80cases).The differences of platelet parameters,five items of blood coagulation and thromboelastography indexes between the two groups were compared.Logistic regression analysis was used to analyze the risk factors of T2DM complicated with DPN.Results:The Angle in the DPN group was 73.50(66.95-75.63)°higher than 67.10(63.50-68.50)°in the simple T2DM group(P<0.05).The MA value of the DPN group was 67.70(63.08-74.88)mm higher than 59.50(54.95-61.70)mm in the simple T2DM group(P<0.05).The Logistic regression analysis results showed that the Angle and MA values were independent risk factors for DPN(OR=0.618,0.683,P<0.05).Conclusion:Angle and MA value of patients with DPN are increased,both of which are independent risk factors for DPN.Angle angle and MA value may be serological markers for diagnosis of DPN.
