基于政策工具的三明市三医联动综合改革政策分析

摘　　要：目的 从政策工具视角探讨三明市三医联动综合改革的特点,为推广三明经验提供借鉴。方法 采用内容分析法,构建政策工具为X维度、医疗医保医药为Y维度的二维分析框架,对2012年1月—2022年3月三明市公开发表的63篇三医联动综合改革政策文本进行量化分析。结果 共计编码889条,发现环境型政策工具使用频率最高(471条),医疗领域政策工具数量最多(493条),医疗、医保、医药领域政策工具结构存在明显不同。结论 三明市三医联动综合改革以环境型政策工具为主,侧重策略性措施、监督监管等政策工具的使用,重视医疗医保医药间的政策工具协同作用,且政策工具偏传统型。建议全国各地借鉴三明经验时,重视使用环境型政策工具营造良好改革氛围、灵活调整政策工具结构、遵循三医联动内在规律,合理使用政策工具并适当进行政策工具创新。Objective Exploring the features of medical-insurance-medicine linkage reform in Sanming from the perspective of policy tools and providing references for the nationwide. Methods Based on content analysis, constructing a two-dimensional analysis framework for content analysis by policy tools as X dimension and medical-insurance-medicine as Y dimension and conducting a quantitative analysis on 63 policy documents published from January 2012 to March 2022 on the medical-insurance-medicine linkage reform in Sanming. Results By encoding889 items,it found that environmental policy tools were the most used,and the number of policy tools targeted at the medical was much more than the two others,the structure of policy tools in these three fields was quite different.Conclusion Sanming City’s medical-insurance-medicine linkage reform is based on environmental policy tools,focusing on the use of strategic measures,supervision and regulation and other policy tools,and emphasizing the synergy of policy tools between medical services,health insurance and medicine,and the policy tools are traditional.It is recommended that when learning from Sanming’s experience,it should pay attention to the use of environmental policy tools to create a favorable reform atmosphere,flexibly adjust the structure of policy tools,reasonably use policy tools according to the inherent rules of the medical-insurance-medicine linkage,and appropriately innovate policy tools in China.
