摘  要:目的:观察加味参芪地黄汤对早期糖尿病肾病患者肾功能及微循环的影响。方法:选择114例糖尿病肾病患者为研究对象,按随机数字表法分为对照组和研究组各57例。对照组给予常规对症治疗,研究组在对照组基础上给予加味参芪地黄汤治疗。评价2组临床疗效,比较2组治疗前后一氧化氮(NO)、血栓素-B2 (TX-B2)、细胞间黏附因子-1 (ICAM-1)、肌酐(SCr)、血尿素氮(BUN)、红细胞聚集指数、纤维蛋白原、血浆黏度、糖化血红蛋白(HbA1c)、游离8-异前列腺素F2α (8-isoPGF2α)水平。结果:研究组总有效率为96.49%,高于对照组80.70%(P<0.05)。治疗后,2组8-isoPGF2α、ICAM-1、BUN、SCr、HbA1c、TX-B2水平较治疗前降低(P<0.05),且研究组8-isoPGF2α、ICAM-1、BUN、SCr、HbA1c、TX-B2水平低于对照组(P<0.05)。治疗后,2组NO水平较治疗前升高(P<0.05),研究组NO水平高于对照组(P<0.05)。治疗后,2组红细胞聚集指数、血浆黏度、纤维蛋白原较治疗前降低(P<0.05),且研究组红细胞聚集指数、血浆黏度、纤维蛋白原低于对照组(P<0.05)。结论:加味参芪地黄汤治疗早期糖尿病肾病患者疗效显著,可降低患者氧化应激及炎症反应,改善肾功能及血液流变学,提升血管内皮功能,促进微循环。Objective:To observe the effect of modified Shenqi Dihuang Decoction on kidney function and microcirculation in patients with early diabetic nephropathy. Methods:A total of 114 cases of patients with early diabetic nephropathy were selected as the study subjects,and divided into the control group and the study group according to the random number table method,with 57 cases in each group. The control group was given routine symptomatic treatment, and the study group was additionally treated with modified Shenqi Dihuang Decoction based on the treatment of the control group. The clinical effects in the two groups were evaluated. Before and after treatment,the levels of nitric oxide(NO),thromboxane B2(TX-B2),intercellular adhesion molecule-1(ICAM-1),serum creatinine(SCr),blood urea nitrogen(BUN),red cell aggregation, fibrinogen, plasma viscosity, hemoglobin A1c(HbA1c) and 8-Isoprostaglandin F2alpha(8-isoPGF2α) in the two groups were compared.Results:The total effective rate was 96.49% in the study group,higher than that of 80.70% in the control group(P<0.05). After treatment,the levels of 8-isoPGF2α,ICAM-1,BUN,SCr,HbA1c and TX-B2 in the two groups were decreased when compared with those before treatment(P<0.05),and the levels of 8-isoPGF2α,ICAM-1,BUN,SCr,HbA1c and TX-B2 in the study group were lower than those in the control group(P<0.05). After treatment,the NO levels in the two groups were increased when compared with those before treatment(P<0.05),and the NO level in the study group was higher than that in the control group(P<0.05). After treatment,the red cell aggregation,plasma viscosity and fibrinogen in the two groups were decreased when compared with those before treatment(P<0.05),and the red cell aggregation,plasma viscosity and fibrinogen in the study group were lower than those in the control group(P<0.05).
