摘　　要：目的探讨超声引导下臂丛神经上干阻滞在肩关节镜手术中的应用效果。方法106例肩关节镜手术患者随机分为臂丛神经组(超声引导下肌间沟臂丛神经阻滞)与神经上干组(超声引导下臂丛神经上干阻滞),比较两组的麻醉效果、术后疼痛情况、术后并发症以及肩臂功能恢复时间。结果两组的麻醉总有效率、术后VAS评分比较,差异均无统计学意义(P>0.05)。神经上干组的肩臂运动功能、感觉功能恢复时间均短于臂丛神经组,术后并发症发生率低于臂丛神经组(P<0.05)。结论超声引导下臂丛神经上干阻滞应用于肩关节镜手术有利于肩臂功能恢复,减少麻醉相关并发症。Objective To explore the application effect of ultrasound-guided brachial plexus upper trunk block in shoulder arthroscopy surgery.Methods 106 patients with shoulder arthroscopy surgery were randomly divided into the brachial plexus group(ultrasound-guided interscalene brachial plexus block)and upper nerve trunk group(ultrasound-guided upper brachial plexus block).The anesthetic effect,postoperative pain,postoperative complications and recovery time of shoulder-arm function were compared between the two groups.Results No significant difference was found in the total effective rate of anesthesia and postoperative VAS scores between the two groups(P>0.05).The recovery time of shoulder-arm motor function and sensory function in the upper nerve trunk group were shorter than those in the brachial plexus group,and the incidence of postoperative complications was lower than that in the brachial plexus group(P<0.05).Conclusions Ultrasound-guided brachial plexus upper trunk block applied in shoulder arthroscopy surgery is conducive to the recovery of shoulder-arm function and reduce anesthesia-related complications.
