心理痛苦温度计在社区2型糖尿病患者中的应用

摘　　要：目的运用心理痛苦温度计(distress thermometer,DT)分析社区2型糖尿病(type 2 diabetes,T2DM)患者心理痛苦状况及其影响因素,为临床诊疗提供理论依据,进而提升患者生存质量。方法随机抽取2021年8月至2022年1月期间,在马鞍山市人民医院全科医学科及其附属社区医院就诊200名T2DM患者进行问卷调查,运用卡方检验和Logistic回归分析、Spearman相关分析进行数据分析。结果社区T2DM患者心理痛苦得分为(4.57±1.88)分,性别、治疗方式、担忧、抑郁、排尿改变、手/脚麻木、孤独、皮肤干燥、头晕、消化不良、疲乏是T2DM患者显著心理痛苦的危险因素(P<0.05)。结论DT较糖尿病痛苦量表(diabetes distress scale,DDS)更适用于社区T2DM患者心理状况的评估,它可以早期识别、全面分析患者的心理痛苦,为后续诊疗提供方向。Objective Distress thermometer(DT)was used to analyze the psychological distress of type 2 diabetes patients in the community and its influencing factors,so as to provide theoretical basis for clinical diagnosis and treatment,and then improve the quality of life for patients.Methods 200 patients with type 2 diabetes who visited the general practice department of Maanshan people′s Hospital and its affiliated community hospitals from August 2021 to January 2022 were randomly selected to conduct a questionnaire survey.Chi-square test,Logistic regression analysis and Spearman correlation analysis were used for data analysis.Results The score of psychological distress in community T2DM patients was(4.57±1.88).Gender,treatment method,worry,depression,urination change,numbness of hands/feet,loneliness,dry skin,dizziness,dyspepsia and fatigue were the risk factors of significant psychological distress in T2DM patients(P<0.05).Conclusion DT is more suitable than diabetes distress scale(DDS)for assessing the psychological status of T2DM patients in the community.It can identify and comprehensively analyze the psychological distress of patients in the early stage,and provide direction for subsequent diagnosis and treatment.
