儿童卵圆孔未闭介入治疗21例临床分析

摘　　要：目的分析儿童卵圆孔未闭的临床表现及介入治疗的适应证和治疗效果。方法选择该院2017年1月至2019年12月因卵圆孔未闭接受介入治疗的21例患儿,分析其临床表现和介入治疗效果。结果21例术后1周、1个月、3个月、6个月、12个月随访,经胸超声心动图检查显示右心室内径、右心房左右径及右心房前后径较介入前降低,差异有统计学意义(P<0.05)。经治疗,9例偏头痛患儿有7例症状消失,6例胸闷心悸患儿症状完全消失。结论介入治疗儿童卵圆孔未闭的疗效及安全性良好,操作技术易于推广,值得临床推荐。Objective To analyse the clinical manifestations of patent foramen ovale(PFO)in children and the indications of interventional therapy and its therapeutic effect.Methods A total of 21 pediatric cases of PFO treated by interventional therapy in Linyi People′s Hospital from January 2017 to December 2019 were selected.The patients′clinical manifestations and the effect of interventional therapy were analyzed.Results The results of transthoracic echocardiography(TTE)showed that compared with those before intervention,the inner diameter of the right ventricle(RV)and the mediolateral diameter and anteroposterior diameter of the right atrium were decreased at the time of follow-up of 1 week,1 month,3 months,6 months and 12 months after surgery,and the differences were statistically significant(P<0.05).The symptoms disappeared in 7 of 9 children with migraine,and the symptoms of chest tightness and palpitation in 6 cases were completely disappeared.Conclusion The interventional therapy is effective and safe for children with PFO.The operation technique is apt to be used widely and worthy of clinical recommendation.
