摘　　要：目的 分析国家药品集中采购对天津市北辰医院门诊降压药使用情况的影响。方法 提取国家药品集中采购政策实施前后门诊降压药的用药数据,采用药物经济学方法回顾性分析药品价格、数量、金额、用药频度(DDDs)、日均费用(DDC)和换药率等变化情况。结果 2018—2022年天津市北辰医院门诊降压药共涉及9个品种,集采药较原用药相比,降价幅度均在50%以上,销售金额减少132.48万元,同比降低54.26%,DDDs同比升高29.01%,DDC均有不同程度的降低。70.71%~100.00%患者将非集采药更换成集采药使用。结论 国家药品集中采购政策切实降低了天津市北辰医院高血压患者用药负担,提高了医保资金使用效率,同时医生和患者越来越倾向于使用集采药,集采药逐步得到大众认可。Objective To analyze the influence of national centralized drug procurement on the use of antihypertensive drugs in outpatient department of Tianjin Beichen Hospital. Methods The data of antihypertensive drugs used in outpatients before and after the implementation of national centralized drug procurement policy were extracted, and the changes of drug price, quantity, amount,DDDs, DDC and dressing change rate were analyzed retrospectively by pharmacoeconomics method. Results From 2018 to 2022, 9varieties of antihypertensive drugs were involved in the outpatient department of Tianjin Beichen Hospital. Compared with the original drugs, the price reduction of collected drugs was more than 50%, the consumption sum decreased by 1 324 800 yuan, 54.26% lower than that of the previous year, DDDs increased by 29.01%, and DDC decreased to varying degrees. 70.71% — 100.00% of patients changed the uncollected medicine into the collected medicine. Conclusion The national policy of centralized drug procurement has effectively reduced the burden of medication for hypertension patients and improved the efficiency of medical insurance funds. At the same time, doctors and patients are more and more inclined to use collection medicine, and collection medicine is gradually recognized by the public.
