摘  要:目的:观察下肢等速肌力训练联合主动呼吸循环技术(ACBT)对慢性阻塞性肺疾病(COPD)患者肺功能和肌力的影响。方法:采用随机数字表法将60例COPD患者分为治疗组及对照组各30例,在常规干预基础上,对照组进行ACBT治疗,治疗组在对照组基础上增加下肢等速肌力训练,于治疗前、治疗8周后检测两组患者肺功能指标(FEV_(1)、FEV_(1)/FVC)和下肢膝关节屈伸肌峰力矩(伸肌PT、屈肌PT),并评估6MWT的改善情况。结果:干预前,两组FEV_(1)、FEV_(1)/FVC、伸肌PT、屈肌PT、6MWT等指标差异无统计学意义(P>0.05);干预8周后,两组FEV_(1)、FEV_(1)/FVC、伸肌PT、屈肌PT较干预前显著改善,且治疗组优于对照组(P<0.05);两组干预8周后6MWT较干预前显著升高,且治疗组高于对照组(P<0.05)。结论:等速肌力训练联合主动呼吸循环技术对COPD患者的肺功能和下肢肌力的改善具有协同作用,能够改善患者的步行耐力,值得临床推广应用。Objective:To observe the effect of isokinetic muscle strength training combined with active respiratory and circulatory technology(ACBT) on pulmonary function and muscle strength in patients with chronic obstructive pulmonary disease(COPD).Methods:60 patients with COPD were randomly divided into treatment group(n=30) and control group(n=30).On the basis of routine intervention,the control group was given ACBT treatment,while treatment group combined with isokinetic muscle strength training of lower limbs on the basis of the control group;The improvement of pulmonary function index(FEV_(1),FEV_(1)/FVC) and peak torque of knee joint flexor and extensor(extensor PT,flexor PT) in both groups were detected before and after 8 weeks of treatment;At the same time,the improvement of 6MWT was evaluated.Results:Before intervention,there was no significant difference in FEV_(1),FEV_(1)/FVC,PT of extensor,PT of flexor,6MWT between the two groups(P>0.05);After 8 weeks of intervention,FEV_(1),FEV_(1)/FVC,PT of extensor and PT of flexor in the two groups were significantly improved compared with those before intervention,and the treatment group was better than the control group(P<0.05);After 8 weeks of intervention,the 6MWT of the two groups was significantly higher than that before intervention,and the treatment group was higher than the control group(P<0.05).Conclusion:Isokinetic muscle strength training combined with active respiratory and circulatory technology has a synergistic effect on the lung function and lower limb muscle strength of patients with COPD,and can improve the walking endurance of patients.The combined therapy is worthy of clinical promotion and application.
