化瘀利水方联合镇痛药对尿道下裂术后疼痛患儿的效果

摘　　要：目的 观察化瘀利水方联合镇痛药对尿道下裂术后疼痛患儿的疗效及对白细胞介素-6(interleukin-6,IL-6)、肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)水平的影响。方法 选取80例行尿道下裂术患儿,按照术后用药方案分为2组。对照组给予单纯药物镇痛,中药组给予化瘀利水方联合药物镇痛。统计2组并发症的发生,记录2组视觉模拟评分(visual analogue score, VAS)、尿动力学指标、IL-6和TNF-α水平。结果 术后1 h 2组VAS评分比较,差异无统计学意义;术后12、24、48、72 h时,中药组VAS低于对照组(P<0.05)。术后24、72 h时,2组IL-6、TNF-α水平均较术前升高,中药组术后24、72 h时IL-6、TNF-α水平低于对照组(P<0.05)。术后3个月,2组最大尿流率(maximum urinary flow rate,Qmax)较术前下降,平均尿流率(average urinary flow rate,Qavc)、排尿量(urination volume,V)、膀胱残余尿量(residual urine volume of bladder, PVR)与术前比较,差异无统计学意义。术后6个月,中药组V较术前升高,Qmax、Qavc和PVR与术前比较,差异无统计学意义;对照组尿动力学指标与术前比较,差异无统计学意义;中药组和对照组术后3、6个月时尿动力学指标比较,差异无统计学意义。中药组并发症率低于对照组(P<0.05)。结论 化瘀利水方联合镇痛药物可减轻尿道下裂术后患儿的疼痛症状,抑制IL-6、TNF-α水平的表达,减少并发症的发生。Objective To observe the improvement effect of Huayu Lishui Decoction combined with analgesic drugs on the level of interleukin-6(IL-6) and tumor necrosis factor-α(TNF-α) in children with hypospadias postoperative pain.Methods 80 children with hypospadias were selected and divided into 2 groups according to the postoperative medication regimen. The control group was given simple drug analgesia, while the Chinese medicine group was given Huayu Lishui Decoction combined with drug analgesia. The complications of the 2 groups were counted, and the visual analogue score(VAS), urodynamic indexes, IL-6 and TNF-α levels of the 2 groups were recorded.Results There was no significant difference in VAS between the 2 groups 1 hour after operation, and the VAS of Chinese medicine group was lower than that of control group. After 24 hours and 72 hours, the IL-6、TNF-α of the 2 groups were higher than that of the control group(P<0.05). The maximum urinary flow rate(Qmax) in the 2 groups decreased compared with that before 3 months after operation. The average urinary flow rate(Qavc), urine volume(V), and bladder residual urine volume(PVR) were not significantly different compared with preoperative. 6 months after operation, the V of traditional Chinese medicine group was higher than that of preoperation, the Qmax, Qavcand PVR was not significantly different compared with preoperation. The urodynamic indexes of Chinese medicine group and control group at 3 and 6 months after operation showed no significantly different. Complications in Chinese medicine group were lower than those in control group.Conclusion Huayu Lishui Decoction combined with analgesic drugs can relieve pain, inhibit the expression of IL-6, TNF-α and reduce the complications after hypospadias.
