摘 要:本文旨在探讨铜绿假单胞菌(Pseudomonas aeruginosa)中exoU与lasI基因的表达及耐药特点。样本选取2019年6月—2020年12月邢台医学高等专科学校第二附属医院收集和分离的640株铜绿假单胞菌,主要来自痰液(51.56%)、血液(22.19%)及脓液(15.16%)标本。结果显示,exoS+/exoU-铜绿假单胞菌在各标本中检出率最高。携带exoU的铜绿假单胞菌对亚胺培南、氨基糖苷类、氟喹诺酮类、大环内酯类抗菌药物均具有较高的耐药率(P<0.001),而lasI缺失的铜绿假单胞菌对各类抗菌药物的耐药率则显著降低(P<0.001)。携带exoU或exoS基因的铜绿假单胞菌转录调控相关基因ptrA、exsA的表达与exoU或exoS的表达呈显著负相关(P<0.001)。lasI缺失的铜绿假单胞菌下游毒力基因lasA、aprX的表达水平显著降低(P<0.001),且lasA、aprX的表达水平与lasI呈显著正相关(P<0.001)。结果提示,exoU基因表达与铜绿假单胞菌的耐药性显著相关,而ptrA可能是exoU的潜在负调控基因,lasI缺失明显抑制铜绿假单胞菌群体感应系统毒力基因的表达及耐药性的产生。The present paper aims to investigate the expression of exoU and lasI genes and drug resistance of Pseudomonas aeruginosa(P. aeruginosa). A total of 640 P. aeruginosa strains were collected from the Second Affiliated Hospital of Xingtai Medical College during June 2019 and December 2020. The strains were mainly from sputum(51.56%), blood(22.19%) and pus(15.16%). The results showed that the detection rate of exoS+/exoU-strains were highest in all specimens. The resistance rates of P. aeruginosa carrying exoU to imipenem, aminoglycosides, fluoroquinolones and macrolides were higher(P<0.001), and the resistance rates of P. aeruginosa with lasI deficiency to all antibiotics were significantly decreased(P<0.001). In P. aeruginosa carrying exoU or exoS, the expression levels of regulatory genes ptrA and exsA were negatively correlated to exoU or exoS(P<0.001). The expression levels of downstream virulence genes lasA and aprX were significantly decreased(P<0.001) after the deletion of lasI gene, and they were positively correlated to lasI expression level(P<0.001). It is confirmed that the expression of exoU was significantly correlated to the drug resistance of P. aeruginosa, and ptrA may be a potentially negative regulatory gene of exoU. The deletion of lasI gene can significantly inhibit the virulence gene expression and drug resistance of P. aeruginosa.
