摘  要:目的 探讨富含丝氨酸/精氨酸剪接因子6(SRSF6)、微小RNA-506-3p(miR-506-3p)在结核性胸膜炎胸腔积液中表达的意义及诊断价值。方法 选取2020年1月到2022年1月本院收治的60例结核性胸膜炎胸腔积液患者作为结核组,另选取同期非结核性胸膜炎胸腔积液患者30例作为对照组。两组患者均于无菌条件下进行胸腔穿刺,抽取胸腔积液进行miR-506-3p、SRSF6水平测定。Pearson法分析结核组SRSF6与miR-506-3p水平的相关性。采用多因素Logistic回归分析影响结核性胸膜炎发生的危险因素。采用受试者工作特征(ROC)曲线分析胸腔积液SRSF6和miR-506-3p水平对结核性胸膜炎的诊断价值。结果 结核组胸腔积液SRSF6表达水平明显高于对照组(P<0.05),miR-506-3p表达水平明显低于对照组(P<0.05)。Pearson相关性分析显示,结核组胸腔积液SRSF6与miR-506-3p表达水平呈负相关(P<0.05)。多因素Logistic回归分析显示,SRSF6高水平、miR-506-3p低水平是影响结核性胸膜炎发生的危险因素(P<0.05)。SRSF6、miR-506-3p联合诊断结核性胸膜炎的价值高于SRSF6及miR-506-3p单独诊断。结论 SRSF6在结核性胸膜炎胸腔积液中表达水平升高,miR-506-3p在结核性胸膜炎胸腔积液中表达水平降低,两者是影响结核性胸膜炎发生的危险因素,两者联合对于结核性胸膜炎具有较高的诊断价值。Objective To investigate the significance and diagnostic value of the expression of serine/arginine-rich splicing factor 6(SRSF6) and microRNA-506-3p(miR-506-3p) in the pleural effusion of tuberculous pleurisy. Methods From January 2020 to January 2022, 60 patients with tuberculous pleurisy and pleural effusion admitted to our hospital were taken as the tuberculosis group. Another 30 patients with nontuberculous pleurisy pleural effusion were regarded as the control group. Both groups underwent thoracentesis under aseptic conditions, and pleural effusion was extracted for the determination of miR-506-3p and SRSF6. The correlation between SRSF6 and miR-506-3p expression in the tuberculosis group was analyzed by the Pearson method. Multivariate logistic regression was applied to analyze the risk factors for tuberculous pleurisy. The receiver operating characteristic(ROC) curve was applied to analyze the diagnostic value of pleural effusion SRSF6 and miR-506-3p levels in tuberculous pleurisy. Results The expression level of SRSF6 in pleural effusion in the tuberculosis group was greatly higher than that in the control group(P<0.05), while the expression level of miR-506-3p was greatly lower than that in the control group(P<0.05). Pearson correlation analysis showed that SRSF6 in pleural effusion in the tuberculosis group was negatively correlated with the expression level of miR-506-3p(P<0.05). Multivariate logistic regression analysis showed that a high level of SRSF6 and a low level of miR-506-3p were risk factors for tuberculous pleurisy(P<0.05). The value of SRSF6 and miR-506-3p combined diagnosis of tuberculous pleurisy was higher than that of SRSF6 and miR-506-3p alone(P<0.05).Conclusion The expression level of SRSF6 in tuberculous pleuritis pleural effusion is increased,and the expression level of miR-506-3p in tuberculous pleuritis pleural effusion is decreased.Both are risk factors for the occurrence of tuberculous pleurisy,and the combination of the two has a high diagnostic value for tuberculous pleurisy.
