下肢浅静脉曲张射频消融联合泡沫硬化术治疗的47例报告

摘　　要：目的评价下肢浅表静脉曲张的下肢经超声引导下行大隐静脉主干射频消融术联合小腿表浅静脉泡沫硬化术后的疗效。方法对2019年1—10月47例非继发性大隐静脉曲张患者,共64条患肢的大隐静脉主干均采用超声引导下射频静脉闭合系统治疗,64条小腿属支浅表曲张静脉使用泡沫硬化治疗,采用临床-病因-解剖-病理生理学分类系统(CEAP分类系统)、Villalta量表、静脉临床严重程度评分表(venous clinical severity score,VCSS)进行术前、术后评分。结果随访6~15个月,平均(9.87±2.37)个月。所有患肢CEAP临床分级、Villalta、VCSS评分术后较术前降低,差异有统计学意义(Z=-4.848、-6.964、-6.771,P均<0.01)。结论大隐静脉主干射频消融联合小腿浅表曲张静脉泡沫硬化剂治疗,短期效果确切。Objective To evaluate the efficacy of ultrasound-guided radiofrequency ablation(RFA)of the main saphenous vein in the lower extremities combined with foam sclerosis of the superficial calf vein for superficial varicose veins of the lower extremities.Methods From January to October 2019,for 47 patients with non-secondary great saphenous varicose veins,a total of 64major saphenous veins of the affected limbs were treated with an ultrasound-guided radiofrequency vein closure system,and 64 superficial varicose veins of the lower legs were treated with foam sclerotherapy.clinical-etiology-anatomy-pathophysiology(CEAP),Villalta,venous clinical severity score(VCSS)were scored before and after surgery.Results The follow-up period was 6-15 months,with an average of(9.87±2.37)months.The CEAP clinical grade,Villalta and VCSS scores of all affected limbs decreased significantly after operation compared with those before operation(Z=-4.848,-6.964,-6.771,P<0.01).Conclusion Radiofrequency ablation of the main saphenous vein combined with foam sclerotherapy for superficial varicose veins of the calf has a definite short-term effect.
