维生素D支持对老年2型糖尿病合并轻度行为损害患者的临床观察

摘　　要：目的研究维生素D(vitamin D,VD)支持对老年2型糖尿病(diabetes mellitus type 2,T2DM)患者轻度行为损害(mild behavioral impairment,MBI)进展,以及血液因子[脑源性神经营养因子(brain-derived neurotrophic factor,BDNF)、同型半胱氨酸(homocysteine,Hcy)]的影响。方法从2019年1月—2020年12月收治的老年T2DM患者中筛出48例合并MBI的患者,将其通过随机数字表法分为干预组和对照组,对照组进行常规治疗,干预组在此基础上给予VD支持干预。在基线,6、12和18个月时,分别检测患者的血清BDNF、Hcy水平,并进行MBI-C量表评分。结果干预组BDNF水平在干预后显著高于对照组,干预组Hcy水平在干预后显著低于对照组,干预组MBI-C评分在12、18个月时,显著低于对照组。结论对老年T2DM患者给予长期的VD支持干预能够显著降低其MBI的发生率和严重程度。Objective To study the effect of vitamin D support in treatment of elderly type 2 diabetes mellitus,patients with mild behavioral impairment(MBI)and blood factors[brain-derived neurotrophic factor(BDNF),homocysteine(Hcy)].Methods A total of 48 elderly type 2 diabetes mellitus patients with MBI were screen out fromJanuary 2019 to December 2020 and divided into the control group and the intervention group accord ingtotherandomnumbertablemethod.The control group received normal diabetes treatment,the intervention group received additional vitamin D(VD)suppose.The MBI-C,BDNF and Hcy levels at baseline,6-month,12-month and 18-month after intervention were measured,and the MBI-C scale was scored.Results After intervention,the BDNF level in intervention group was significantly higher than that in control group,the Hcy level in intervention group was significantly lower than that in control group,and the MCI-C score in intervention group was significantly less than that in control group at 12-month and 18-month.Conclusion Long-term VD support to elderly type 2diabetes mellitus patients can significantly reduce the incidence and severity of MBI.
