摘　　要：目的探讨采用解剖型锁定钢板治疗后Pilon骨折(PPF)的临床疗效。方法选取2016年6月至2020年6月该院收治的PPF患者20例,术前均有明确踝关节旋转-轴向暴力受伤机制。均采用后外侧入路治疗,取跟腱和腓骨肌间隙进入,保护腓肠神经,采用解剖型锁定钢板内固定治疗PPF。术后均在医师规范指导下进行患侧踝关节功能康复锻炼。采用美国足踝外科协会踝-后足功能评价系统评价踝关节功能。随访时间12~20个月,平均(15.6±2.76)个月。结果20例患者中踝关节功能优16例,良4例。结论后外侧入路解剖锁定钢板内固定治疗PPF临床疗效佳,并发症少,可以早期负重功能锻炼。Objective To retrospectively analyze the clinical efficacy of Pilon fractures treated with anatomic locking plate.Methods Twenty cases of posterior Pilon fractures treated in this hospital from June 2016 to June 2020 were selected in this study,and the mechanism of ankle joint rotation-axial violence injury was clarified before operation.The treatment methods were all adopted the posterolateral approach,entered the space between the Achilles tendon and the peroneal muscle,protected the sural nerve,and used anatomical locking plate internal fixation to treat the posterior Pilon fracture.The American Orthopedic Foot and Ankle Society(AOFAS)ankle and hindfoot function scores were used to evaluate.Results All patients were followed up clinically,and all patients underwent rehabilitation exercises for the ankle joint function of the affected side under the standard guidance of physicians after operation.AOFAS ankle-hindfoot function evaluation:excellent in 16 cases,good in 4 cases.Conclusion The internal fixation of Pilon fractures with anatomic locking plate via posterolateral approach has good clinical curative effect and fewer complications,and early weightbearing functional exercise can be performed.
