钓鱼式结扎方式在腹腔镜阑尾炎日间手术中的应用价值

摘　　要：目的探讨钓鱼式结扎方式在腹腔镜阑尾炎日间手术中的应用价值。方法回顾性分析该院收治是52例行日间腹腔镜阑尾切除术患者的临床资料,随机分为观察组和对照组,每组26例。最终符合条件41例,观察组20例,对照组21例。对照组行分段丝线结扎阑尾,器械护套取出阑尾,观察组行钓鱼式结扎阑尾与取出阑尾。比较两组患者临床疗效。两组患者出院后均随访7 d。结果与对照组比较,观察组患者阑尾根部结扎时间、阑尾取出时间均更短,治疗费用更少,差异均有统计学意义(P<0.05)。对照组患者中出现切口疼痛1例,两组患者均无切口感染、发热不适等,治疗情况均较好。结论两种阑尾处理方式在日间手术中均可行且安全,钓鱼式结扎方式适用不同阑尾位置较广。Objective To explore the value of fishing ligation in laparoscopic appendectomy.Methods The data of 52 patients undergoing daytime laparoscopic appendectomy in our hospital were retrospectively analyzed.According to the inclusion and exclusion criteria,41 patients were finally eligible,21 patients in the control group and 20 patients in the observation group.The control group was ligated with segmented silk thread,and the instrument sheath was taken out of the appendix.The observation group was ligated with fishing and the appendix was taken out.Results There was a statistically significant difference between the two groups in the ligation time of appendix root(P<0.05).Compared with the control group,the operation time of the observation group was shorter and the operation time was significantly shorter(P<0.05).With the shortening of the operation time,the treatment cost would also be relatively reduced.The difference in the treatment cost was statistically significant(P<0.05),One patient in the control group had incision pain,and the two groups had no incision infection,fever and discomfort,and the treatment was good.Conclusion The two appendectomy methods are feasible and safe in the daytime operation.The fishing ligation method is widely applicable to different appendages.
