祛瘀化痰汤联合无创正压通气法治疗老年阻塞性睡眠呼吸暂停低通气综合征的疗效评价

摘　　要：目的分析祛瘀化痰汤联合无创正压通气法治疗老年阻塞性睡眠呼吸暂停低通气综合征(OSAHS)的实际临床疗效。方法研究对象选取为2020年1月至2021年5月睢县人民医院收治的88例老年OSAHS患者,利用随机数表方法,将全部患者分为对照组及观察组各44例。对照组给予无创正压通气,观察组给予无创正压通气联合祛瘀化痰汤的方法治疗,两组患者均连续治疗4周,对比两组患者的临床疗效,采用Calgary睡眠呼吸暂停生活质量指数(SAQLI)评价对比治疗前后两组患者的生活质量,采用Epworth嗜睡量表(ESS)和匹兹堡睡眠指数(PSQI)评价对比治疗前后两组患者的睡眠质量。结果观察组治疗有效率[90.91%(40/44)]明显高于对照组[70.45%(31/44)],差异有统计学意义(χ^(2)=6.019,P<0.05);经过治疗后观察组SAQLI评分显著高于对照组,治疗后观察组ESS、PSQI评分均显著低于对照组,差异有统计学意义(t=4.925,-5.049,-7.401,P<0.05)。结论祛瘀化痰汤联合无创正压通气法治疗老年OSAHS的疗效确切,可以提升老年OSAHS患者的睡眠以及生活质量水平,值得临床应用。Objective To analyze the clinical efficacy of Quyu Huatan decoction combined with non-invasive positive pressure ventilation in the treatment of elderly obstructive sleep apnea hypopnea syndrome(OSAHS).Methods The subjects of the study were 88 elderly OSAHS patients admitted to Sui County People’s Hospital from January 2020 to May 2021.All the patients were divided into control group and observation group with 44 cases each by random number table.The control group was treated with non-invasive positive pressure ventilation,and the observation group was treated with non-invasive positive pressure ventilation combined with Quyu Huatan decoction.The two groups of patients were treated continuously for 4 weeks.The clinical efficacy of the two groups of patients was compared,and the quality of life of the two groups of patients before and after treatment was evaluated by the Calgary sleep apnea quality of life index(SAQLI),The sleep quality of patients in the two groups before and after treatment was evaluated and compared with Epworth sleepiness scale(ESS)and Pittsburgh sleep index(PSQI).Results The effective rate of treatment in the observation group[90.91%(40/44)]was significantly higher than that in the control group[70.45%(31/44)],and the difference was statistically significant(χ^(2)=6.019,P<0.05);After treatment,the SAQLI score of the observation group was significantly higher than that of the control group.After treatment,the ESS and PSQI scores of the observation group were significantly lower than that of the control group(t=4.925,-5.049,-7.401,P<0.05).Conclusion Quyu Huatan decoction combined with non-invasive positive pressure ventilation is effective in the treatment of elderly OSAHS,which can improve the sleep and quality of life of elderly OSAHS patients,and is worthy of clinical application.
