摘 要:目的 分析胸水和血清中腺苷脱氨酶(ADA)与乳酸脱氢酶(LDH)水平及比值在结核性胸膜炎诊断中的意义。方法 选取20例结核性胸膜炎患者作为观察组,另选取同期20例肝性胸水患者作为对照组。两组患者均进行胸水和血清中ADA与LDH水平检测,计算其相应的比值。比较两组患者胸水和血清中ADA水平及比值,胸水和血清中LDH水平及比值。结果 观察组患者的胸水腺苷脱氨酶(PADA)(68.50±1.02)U/L及PADA/血清腺苷脱氨酶(SADA)(3.13±1.40)均显著高于对照组的(3.55±0.98)U/L、(0.94±0.45), SADA(16.20±2.80)U/L低于对照组的(24.60±17.59)U/L,差异均有统计学意义(P<0.05)。观察组患者的胸水乳酸脱氢酶(PLDH)(418.00±201.40)U/L和PLDH/血清乳酸脱氢酶(SLDH)(3.44±2.27)均显著高于对照组的(52.50±46.60)U/L、(0.35±0.25), SLDH(203.00±40.70)U/L低于对照组的(254.50±58.00)U/L,差异均有统计学意义(P<0.05)。结论 结核性胸膜炎患者胸水和血清中的ADA、LDH比值显著高于肝性胸水患者,所以胸水和血清中的ADA与LDH比值在结核胸膜炎诊断中具有重要临床意义。Objective To analyze the significance of the levels and ratio of adenosine deaminase(ADA and lactate dehydrogenase(LDH) in pleural fluid and serum in the diagnosis of tuberculosis pleurisy. Methods 20 patients with tuberculous pleurisy were selected as the observation group, and another 20 patients with hepatic hydrothorax in the same period were selected as the control group. Patients in both groups were tested for ADA and LDH levels in pleural fluid and serum, and their corresponding ratios were calculated. The level and ratio of ADA in pleural fluid and serum, and level and ratio of LDH in pleural fluid and serum were compared between the two groups. Results Patients in the observation group had significantly higher pleural adenosine deaminase(PADA)(68.50±1.02) U/L and PADA/serum adenosine deaminase(SADA)(3.13±1.40) than(3.55±0.98) U/L and(0.94±0.45) in the control group, and SADA(16.20±2.80) U/L was lower than(24.60±17.59) U/L in the control group, and all differences were statistically significant(P<0.05). Patients in the observation group had a significantly higher pleural lactate dehydrogenase(PLDH)(418.00±201.40) U/L and PLDH/serum lactate dehydrogenase(SLDH)(3.44±2.27) than(52.50±46.60) U/L and(0.35±0.25) in the control group, and SLDH(203.00±40.70) U/L was lower than(254.50±58.00) U/L in the control group, and all differences were statistically significant(P<0.05). Conclusion The levels and ratios of ADA and LDH in pleural fluid and serum in patients with tuberculous pleurisy are higher than those in hepatic hydrothorax patients. Therefore, the ratios of ADA and LDH in pleural fluid and serum is of great clinical value in the diagnosis of tuberculous pleurisy.
