基于数据挖掘探讨针灸治疗多囊卵巢综合征伴胰岛素抵抗的选穴规律

摘　　要：目的总结针灸治疗多囊卵巢综合征(PCOS)伴胰岛素抵抗(IR)的选穴规律。方法检索中国知网、维普网、万方数据知识服务平台、中国生物医学文献服务系统及PubMed数据库,按照纳入与排除标准筛选针灸治疗PCOS伴IR的文献,建立针灸处方数据库。利用Microsoft Excel软件进行描述性分析,运用SPSS Modeler 14.1、SPSS Statistics 20.0对使用频次≥10次的腧穴进行关联规则分析和聚类分析。结果经筛选共纳入101篇文献,涉及腧穴81个。针灸治疗PCOS伴IR使用频次最高的腧穴为关元、三阴交、中脘、天枢、足三里等;常用腧穴分布于任脉、足阳明胃经、足太阴脾经;特定穴以交会穴、募穴、五输穴为主。关联规则分析结果显示,24组穴位呈显著正相关(均P<0.05),其中肝俞-膈俞-脾俞-肾俞-卵巢-水分、天枢-带脉-丰隆-阴陵泉-水分、下脘-中脘-气海-水道组合的关联度较高。聚类分析得到3类腧穴组合:(1)气海、中脘、天枢;(2)归来、太溪、大横、水道、下脘、太冲、血海、带脉、阴陵泉、卵巢、水分、膈俞、肝俞、脾俞、肾俞、中极;(3)子宫、关元、三阴交、丰隆、足三里。结论针灸治疗PCOS伴IR的选穴多以补脏腑之虚、调脾胃之功能、利水液之代谢为原则,其腧穴配伍体现了标本兼治、补虚泻实的治疗原则。Objective To summarize the acupoint-selected rules of acupuncture and moxibustion for the treatment of polycystic ovarian syndrome(PCOS)with concomitant insulin resistance(IR).Methods The databases of China National Knowledge Infrastructure,Weipu,Wanfang,China Biology Medicine,and PubMed were retrieved.The literature of acupuncture and moxibustion for the treatment of PCOS and concomitant IR were screened according to inclusion and exclusion criteria,and the prescription database of acupuncture and moxibustion was established.Descriptive analysis was performed by the Microsoft Excel software.The SPSS Modeler 14.1 and SPSS Statistics 20.0 were used for analyses of associated rule and cluster on acupoint with use frequency≥10.Results A total of 101 literature involving 81 acupoints were included after screening.The acupoints used most frequently for treating PCOS and concomitant IR were Guanyuan(CV4),Sanyinjiao(SP6),Zhongwan(RN12),Tianshu(ST25),and Zusanli(ST36),etc.,and the common used acupoints were distributed in Conception Vessel,Stomach Meridian of Foot-Yangming,Spleen Meridian of Foot-Taiyin;in addition,the specific acupoints were mainly Crossing point,Front-mu point,and Five-shu point.The results of associated rule analysis revealed that significantly positive correlation was interpreted in acupoints of 24 groups(all P<0.05),therein the combinations with respect to Ganshu(BL18)-Geshu(BL17)-Pishu(BL20)-Shenshu(BL23)-Luanchao(MA)-Shuifen(RN9),Tianshu(ST25)-Daimai(GB26)-Fenglong(ST40)-Yinlingquan(SP9)-Shuifen(RN9),and Xiawan(RN10)-Zhongwan(RN12)-Qihai(RN6)-Shuidao(ST28)implied a relatively higher correlation degree.The cluster analysis obtained three combinations of acupoints as follows:(1)Qihai(RN6),Zhongwan(RN12),and Tianshu(ST25).(2)Guilai(ST29),Taixi(KI3),Daheng(SP15),Shuidao(ST28),Xiawan(RN10),Taichong(LR3),Xuehai(SP10),Daimai(GB26),Yinlingquan(SP9),Luanchao(MA),Shuifen(RN9),Geshu(BL17),Ganshu(BL18),Pishu(BL20),Shenshu(BL23),and Zhongji(RN3).(3)Zigong(EX-CA1),Guanyuan(CV4),Sanyinjiao(SP6),Fenglong(ST40),
